Occupational Therapist – Professional Home Care Associates – Fremont, CA

Home Care Agency in need of a PART-TIME Occupational Therapist that will cover Alameda County.

Professional Home Care Associates is owned by two Physical Therapists, making patient care our priority.

Essential Function:

  • Accepts assignments and performs care in coordination with case manager and physician.
  • Providing treatment as ordered by the attending physician.
  • Assisting the physician in evaluating level of function.
  • Assisting in developing and updating the plan of treatment.
  • Observing, recording and reporting information on the patient’s condition, to the attending physician and in the patient’s health record.
  • Advising, consulting, and when appropriate, instructing family and other agency personnel, in patient’s therapy program.
  • Teaching and supervising other health personnel when appropriate.
  • Evaluating the home environment and making appropriate recommendations.
  • Participating in in-service education programs.
  • Reports changes in the patient’s condition and needs to the case manager, to the Supervisor and/or the physician.
  • Completes records appropriately
  • Turns paperwork into the office in a timely manner.

EMPLOYMENT STANDARDS:

  • Must have Occupational Therapy License from the State of California
  • Own transportation, Drivers License and Automobile Insurance
  • Excellent oral and written communication skills
  • Strong Occupational Therapy skills
  • Commitment to practice quality service delivery
  • Attends 12 hours of in-service annually

*High flexibility of work schedule

Job Type: Part-time

Pay: $80.00 – $120.00 per hour
