Full-Time (36 hours/week) – Days (8:00am – 4:30pm)

As an advocate for the patient, the Clinic RN – Care Consultant (RN-CC) assists patients to reach optimal function and health in an appropriate environment that would support their needs. The RN-CC functions within the full scope of nursing practice adhering to the WI Nurse Practice Act, Administrative Code of the WI Board of Nursing and/or appropriate nursing standards governing nursing in the state of practice, the ANA Nursing Scope and Standards of Practice and ANA Art and Science of Nurse Coaching, the ANA Code of Ethics for Nurses and Bellin Health Policies and Procedures. Position includes the major responsibilities of the essential job duties and to extend these responsibilities beyond the episode of care to encompass coordination of care across the continuum. The continuum of care includes inpatient to outpatient to community for high risk people. The length of time an RN-CC would follow a patient is determined by patient needs and attainment of mutual goals. Position also includes providing a safe environment for patients, families, staff, and visitors, and working collaboratively with the multi-disciplinary health care team to provide the best outcomes for the patient. Community involvement and collaboration is encouraged.

WI RN License and Bachelor of Science in Nursing (BSN) degree required. The Bachelor’s degree requirement may be waived for existing Diploma and Associate Degree in Nursing (ADN) RNs based on guidelines established by leadership. If the RN holds a Bachelor’s degree other than a BSN; requirements, again, will be determined by leadership. ACLS and CPR certification required. Basic computer and keyboarding skills, excellent verbal and written communication skills, team player, ability to function independently, able to multi-task, organized and sensitive to patient needs are required.

Five years clinical experience, with minimum two years being in an acute setting, required. In addition, experience in care/disease management, health coaching, utilization review, and/or discharge planning preferred. Demonstrated ability of adhering to clinical and regulatory standards, program development, managing department protocols and processes, and managing patient flow required. Five years of experience in cardiac field; proficient knowledge of cardiac medicine required.
