PRIMARY RESPONSIBILITIES AND ESSENTIAL FUNCTIONS:
1. Performs initial resident assessment
2. Identifies resident’s physical needs, psychosocial needs, and problems
3. Assesses resident’s response to medications and/or treatments
4. Monitors closely unstable residents; calls physician when indicated
5. Organizes resident care assignments to meet needs, including deployment of assigned staff
6. Prioritizes needs appropriately
7. Participates in care planning, discharge, admission and other meetings when assigned or requested
8. Administers medications accurately and safely to assigned group of residents
9. Transcribes physician orders correctly and completes necessary paperwork to ensure completion
10. Participates in Quality Assurance performance Improvement (QAPI) activities as assigned by center or department leadership
11. Implements resident education plans when appropriate
12. Delivers appropriate nursing care to individual or groups of residents
13. Utilizes therapeutic verbal and non-verbal interventions with residents
14. Seeks supervisory assistance appropriately
15. Assists residents in preparing for and attending scheduled activities
16. Assumes and carries out responsibility for leadership of nursing care team when assigned
17. Communication with other health care team members such as physicians, nurses, CNAs, social workers, and psychologists regarding resident status and response to treatment
18. Helps provide a safe and suitable environment for residents
19. Makes every effort to maintain residents’ safety, comfort, and peace of mind
20. Respects residents’ rights
21. Assists and teaches the resident to be as self-sufficient as possible
22. Receives report at beginning of shift; gives report at end of shift
23. Counts narcotics and checks lock on emergency box; reports to DON and pharmacy of deficiencies
24. Keeps key to medication cart and narcotic cabinet in his/her possession at all times
25. Orders medication from pharmacy as needed
26. Observes and constantly teaches aides in care of residents for safety and total resident care, in compliance with established policies and procedures and state regulations. Some examples of teachings include the following: personal hygiene, dressings treatments, exercise, changing or emptying of ostomy bags as necessary, keeping residents dry and comfortable, serving of food and feeding residents, monitoring of TPR and blood pressure, infection prevention
27. Practices and oversees that restorative nursing measures are taken
28. Isolates communicable diseases
29. Prevents spread of infection by proper disposal of waste and cleaning of equipment
30. Inserts gastrostomy feeding tubes and feeds residents having these tubes
31. Does tracheostomy care and suctioning of residents
32. Administers oxygen
33. Removes sutures and wire clamps
34. Inserts catheters, irrigates catheters, and monitors intake and output
35. Collects and sends specimens to lab, such as blood, stool, urine, sputum; make out proper lab slip
36. Flags charts to alert physician (s) for signatures.
37. Informs physician of drugs not covered by Medicaid, works with pharmacy and physician to identify an alternative
38. Maintains accurate and complete resident’s records, summaries, care plans, nurse’s notes, doctor’s orders, admission and discharge summaries.
39. Utilizes PCC and POC as required to meet center expectations and supervises input of others
40. Enter physician orders in computer
41. Other duties as assigned

OVERTIME REQUIREMENT
Because our residents require assistance around the clock, the ability to work overtime is a requirement for this position and can include Saturdays and Sundays when necessary.
EDUCATION:
1. Hold a valid license as a Registered Nurse in WV
2. Maintains current CPR certification:
