We are seeking an experienced and dynamic individual for the role of Permanent Healthcare Director Case Management in the Medical Industry. This role is an integral part of our team, playing a pivotal role in the coordination of healthcare services, including planning, organizing, and directing the overall operations of the Case Management department. The candidate will be responsible for leading a team of case managers to deliver the highest standard of patient care, ensuring efficient utilization of healthcare resources, and working collaboratively with physicians, staff, and other healthcare professionals to achieve the best possible patient outcomes.

Responsibilities:

  • Oversee and manage the daily operations of the Case Management department, ensuring that all departmental objectives are met and that the highest standards of patient care are maintained.
  • Develop and implement case management programs, policies, and procedures to ensure the delivery of high-quality, cost-effective healthcare services.
  • Collaborate with physicians, nursing staff, and other healthcare professionals to coordinate patient care, develop care plans, and facilitate the appropriate allocation of healthcare resources.
  • Conduct regular reviews of patient cases to ensure that care plans are being followed and that patient needs are being met.
  • Lead and mentor a team of case managers, providing guidance, support, and performance feedback to promote professional growth and development.
  • Monitor and analyze healthcare utilization trends and implement strategies to improve efficiency and effectiveness.
  • Ensure compliance with all relevant laws, regulations, and industry standards, maintaining up-to-date knowledge of changes and developments in the healthcare field.
  • Participate in strategic planning and decision-making at the management level, contributing to the overall success and growth of the organization.

Qualifications:

  • A minimum of 5 years of experience in case management or a related field within the healthcare industry.
  • Current certification as a Case Management Nurse (ACM Certification) or Certified Case Manager (CCM Certification) is required.
  • Proven experience in utilization review and discharge planning.
  • Strong leadership skills, with the ability to motivate and inspire a team.
  • Excellent organizational skills, with the ability to manage multiple tasks and priorities effectively.
  • Strong analytical skills, with the ability to interpret data and make informed decisions.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with patients, families, and healthcare professionals.
  • A commitment to continuous learning and professional development.
  • Knowledge of relevant laws, regulations, and industry standards related to case management in healthcare.
  • A Bachelor’s degree in Nursing or a related field is required. A Master’s degree is preferred.
