Social Worker 2, Palliative Care – Community Health System – CA – Fresno, CA

Overview:

  • All positions are located in Fresno/Clovis CA *

In this role, you will be responsible for providing psychosocial-emotional support and services to patients and families. At any given time, you will be working with 35-45 patients, meaning that you will always have the opportunity to learn and encounter a variety of patients. Individuals on this team make up half of the case management team, a group of 129 members. Each floor of our facility has 1-3 Social Worker’s on hand at all times, this means you will always be encountering a variety of specialties and types of care. As a member of our team, you will serve as a point of contact in order to assist patients and family in understanding, coping and adapting to the person, emotion and bio-psychosocial issues.

Because of the variety of patients we see, you will be given the constant opportunity for experience and patient engagement. This means you should have the ability and willingness to:

Serves as a preceptor and provides individual and group mentorship for unlicensed Social Workers and social worker interns in accordance with regulations of relevant State of California licensing boards.

Collaborate with multi-disciplinary team in the coordination of care by evaluating, assessing and monitoring the patient care plan.

May provide functional direction to unlicensed social work staff and social work interns.

Participates in the coordination of transfers to alternative level of care as appropriate, including acute psychiatric facilities.

Provide comprehensive assessments and referrals, crisis intervention and care management.

Collaborate with multi-disciplinary team and other internal and external personnel to promote an awareness of environmental and psychosocial needs and to develop and implement treatment plans that support patient-centered plan of care for both individual patients and the medical community.

Have excellent communication and problem-solving skills.

Participates in interdisciplinary planning, case conferences, grant requirements, researches and/or quality improvement projects as needed. Qualifications:

Experience and education minimum requirements:

Master’s Degree in Social Science (MSW) required

1 year of clinical, case management, social work or related experience required

Licenses and Certifications:

BLS – Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required

Preferred Licenses and Certifications:

CISM – Critical Incident Stress Management preferred

ACM – Accredited Case Management Certification preferred

OSW – Oncology Social Worker preferred

CHP-SW – Certified Hospice and Palliative Care Social Worker preferred
