Offsite Patient Coordinator

Reports to: Practice Manager/Practice Medical Director

Job Duties:

  • Checks in and arrives patients for visits
  • Enters and verifies patient data in Practice Management system and EMR: race, ethnicity, circle of care, preferred language, pharmacy info, emergency contact, email address, contact phone numbers, referring MDs info
  • Verifies insurance information
  • Updates any changes in patient demographic and insurance data
  • Answers all telephone inquiries regarding Physicians, Providers, insurance coverages and appointments
  • Obtains valid referrals for patients that will see the specialist. Keeps track of referrals to ensure patent is aware of when a new referral will be needed
  • Enters referral/authorization info in practice management systems
  • Scans referrals into EMR for reference
  • Scans insurance cards, ID, registration form, medical intake history, HCP into EMR for reference
  • Reschedules patient appointments
  • Assures that all non-diabetic patients complete the Pre-diabetic assessment form
  • Schedules follow up appointment for patients as per providers’ requests
  • Takes massages for providers and calls and/or pages them as per protocols
  • Completes “To Do” requests
  • Complete telephone encounters assigned/routed in a timely manner
  • Print Provider schedules daily
  • Clear credit card machines daily and send report to MMS
  • Registers all patients on the patient portal/My Chart
  • Assists in preparation of staff schedules and staffing assignments
  • Coordinates front office functions with clerical staff
  • Ensures efficiency of patient flow with clinical staff and providers
  • Escorts patients to exam rooms as needed
  • Creates online/paper referrals for patients to see specialists
  • Tracks patient referrals to ensure that patients have valid referrals
  • Obtains Prior Authorizations for diagnostic images and procedures
  • Informs patients when referrals/prior authorizations have been obtained and assists with appointments as needed
  • Contacts referring MD offices as needed
  • Sends consultation reports to referring providers in a timely manner
  • Supplies patients with the Obligations for the Medical Home information
  • Contacts and reschedules patients when provider is unavailable
  • Collects copayments/payments and posts them in IDX Check-in function
  • Creates daily copayment/payments batches in IDX reconciles batch at the end of shift
  • Updates appointment status in IDX (pending, arrived) by the end of shift
  • Assists patients that are referred to another provider/facility by providing transition of care summary
  • Completes prescription refill template in eCW for patients requesting medication refills
  • Makes arrangements for a translator for hearing impaired or patients that need language interpretation
  • Provides patients with information on providers being referred to
  • Updates Par8o in a timely manner (by end of shift)
  • Provides after visit summary to every patient
  • Informs patients of provider delays
  • Scans incoming correspondence and routes to the appropriate party in a timely manner
  • Send medical records to requestors within ten business days or as needed
  • Purges old charts and prepares to send to storage
  • Adds postage to outgoing mail and places in mailbox at end of workday
  • Sorts and distributes daily mail delivery
  • Scans additional data from charts into EMR according to scanning policies and protocols
  • Respond and handle “Referral” messages within 24 hours

Skills/Qualifications:

1-3 years experience in medical field

Excellent interpersonal/communication skills

Ability to multi-task in a fast paced environment

Must be comfortable dealing with all levels of management and patients

Knowledge of practice operations

Education: High School Diploma or Equivalent

Job Type: Full-time

Pay: $47,664.00 – $51,280.00 per year
