Case Manager for Special Needs – Lexington Behavioral Health Services – Phoenix, AZ

At Lexington Behavioral Health, we provide in-home/community based one on one psycho-social rehabilitation therapy and behavior interventions for at-risk children / adults (our patient population struggles with both developmental disabilities and behavior health diagnosis). Quality Case supervisors help identify member strengths and needs as well as foster life skills training and social/emotional development. Case Supervisors are responsible for managing coaching teams who work out in the field directly with our members. Coaches work with families and youth in their homes, and out in the community by teaching a variety of skills ranging from skills for daily living to social and emotional self-regulation.

LBHS Case Supervisors are expected to assist coaches and coordinate activities with their team. This coordination will take place both in office, and on site with the members on your caseload. When necessary, case supervisors will be expected to provide support after hours or on weekends. This position is set at 40 hours per week with no overtime expected.

Compensation:

This is a salaried position ranging from $21 per hour or $41,600 per year to $26 per hour or $54,080 per year.

Minimum requirements for this position

  • Must be 21 to apply
  • Must be a current resident of AZ, available for an in-person interview
  • Bachelor’s degree or higher in a related field (to be verified at interview)
  • 3+ years of experience working with developmental disability / special needs
  • 3+ years of experience in supervision or case management
  • Willing to work full time
  • Call out coverage for your coaching team members
  • On call after hours / weekend rotation participation

Case Supervisor / Case Manager job expectations:

  • Adhering to LBHS policies and procedures
  • Dressing appropriately for all situations (Dressed up for meetings, when in-office; casual for observations, especially if going into the community, etc.).
  • Being prepared for weekly clinical meetings.
  • Using professional language and conversation in and out of the office (including how we speak about our members).
  • Submitting clinical and administrative paperwork by due dates
  • Responding appropriately to feedback and maintaining or improving performance accordingly
  • Responding to communication in a timely manner (same day with your coaches)
  • Ensuring client information is always up to date in all systems (notify appropriate person if you can’t update).
  • Assisting the clinical team with conducting assessments
  • Assisting with training coaches, caregivers, and support staff.
  • Ability to self-manage time, tasks, and own schedule.
  • Utilizing LBHS therapeutic stance with staff, members, other providers, caregivers)
  • Utilizing resources (e.g., videos, textbooks, assessment protocols, articles, review CM resources online) to build knowledge.
  • Helping to foster a cooperative environment by stepping in to assist fellow Case Supervisors when needed.

Job Type: Full-time
