The
UWMC Surgical Specialties Center has an outstanding opportunity for a
Patient Care Coordinator (Full Time, Day Shifts). The Patient Care Coordinator (PCC) is responsible for coordinating the delivery of medical care, as formulated by UWMC physicians or their designee, and the patient’s access to various health care services. The PCC will coordinate the patient’s entry into and through the health care system, coordinate patient care services and serve as patient advocate.

REPOSNIBILITIES

Patient Access:

Act as primary liaison for UWMC/UW Medicine and represent departmental physician (s) with the hospital, patient and family, referring physicians, outside agencies, staff from outside hospitals and general public by providing information and advice regarding UWMC hospital services.

Triage medical information in a specific medical or surgical specialty.

Effectively communicate with a variety of Health Care Professionals, patients and their advocates, hospital and clinic personnel both at UWMC and other medical facilities, representatives of insurance companies and other organizations related to the health care industry.

Request interpreters as appropriate for appointments, admissions, and surgery.

Maintain knowledge of billing and medical record documentation requirements, diagnostic tests and procedures and specialized pre-procedure instructions with guidelines set by the department or clinic Chairperson or Medical Director.

Patient Intake/Insurance:

Answer the telephone in a professional and courteous manner and directs calls appropriately.

Responsible for reviewing registration data such as demographic, insurance, referral, next of kin etc., and updating online or providing information to appropriate personnel so that an update may be done.

May provide clinic charges for visit types, procedures, and surgeries to patients or prospective patients to the clinic.

Contact review organizations and insurance companies to ensure prior approval requirements are met before patients enter the system such as pre-authorization and referrals

Scheduling: Clinic

Schedule patient appointments by following the appropriate guidelines by clinic, department and physician. Enter directly into appointment system or triage to appropriate person.

Scheduling: Admission

Responsible for completing an A 49 for all planned hospital admissions or overnight stays

Review registration data with patient for scheduled inpatient surgery/admission or out-patient surgery/procedures.

Responsible for coordination of pre-admission tests and procedures.

Coordinate the direct transfer of patients from outside hospitals to UWMC with physicians, medical staff and admitting through the Patient Flow supervisor.

communicate any cancellations or changes to Admitting, Financial Clearance Center, UWP, Financial Counseling and medical staff.

Scheduling: Surgery

Act as liaison between patient, physicians, OR, nurses, other clinics, nursing units, admitting, Financial Counseling, blood bank, lab, Radiology, OT, PT, Prosthetics, etc. for the purpose of coordinating all pre-op planning and preparation.

Schedule and communicate pre-op planning and preparation to patient. Send appointment and other information to patients.

Responsible for requesting necessary surgical equipment, tools, supplies, devices, and implants

Manage OR block time. Release block time and requests elective OR time through the Surgical Services Website.

Responsible for scheduling, rescheduling, and canceling surgeries on ESI scheduling system.

May assist with completion of surgery packet.

Follow-up Care:

May assist with discharge from clinic or hospital by providing follow-up appointments, arranging transportation or housing.

Requirements:

Bachelor’s Degree in a medically related or public relations field AND TWO years of experience in a tertiary patient care setting involving interaction with a variety of medical services, contact with patients and families, interaction with physicians, and proficiency in medical terminology OR equivalent education/experience.
