The primary purpose of your job position is to perform selected acts for promotion of health and in the care of persons who are ill, injured or experiencing alterations in the normal health process in accordance with the facility mission philosophy and as directed by the Director of Nursing. The Licensed Practical Nurse is responsible for determining the nursing problems for an assigned group of residents, making plans to solve them, initiating the plan or assigning others to implement it, and evaluating the extent to which the plan was effective in the problem resolution. All such nursing care shall be given under the direction of a registered nurse. The LPN may perform charge duties as needed. As a Licensed Practical Nurse, you are delegated the administrative authority, responsibility, and accountability necessary for carrying out your assigned duties.

>MAJOR RESPONSIBILITIES

1. Contributes to the completion of the nursing assessment/MDS at the time of admission and updates the MDS quarterly, annually, and when there has been a significant resident change.

2. Participates in identifying active resident problems requiring nursing intervention at the time of admission and ongoing.

3. Participate in the development of a comprehensive plan of care for identified nursing problems. Evaluates the effectiveness of the nursing interventions. Participate in the revision of the plan of care as indicated by changes in residents’ status.

4. Develops appropriate outcome criteria for each identified nursing problem. Evaluates the attainment of outcome criteria ongoing and at discharge.

5. Supervises personnel with respect to accountability for performance including hiring, training, discipline, counseling performance evaluations, grievance measures, staffing levels, and other related personnel functions in order to improve service levels

6. Encourages attending physicians in the treatment plan reviews, records and signs orders and progress notes in accordance with established facility guidelines.

7. Charts nurses’ notes in an informative and descriptive manner that reflects the care provided to the resident, as well as the resident’s response to the care.

Charts all accident and incidents involving residents. Performs other routine charting duties as required and in accordance with established charting guidelines.9. Ensures that an adequate supply of floor stock medications, supplies and equipment is on hand to meet the nursing needs of the residents.

10. Documents all nursing care delivered to the resident. Delegates certain documentation to other personnel consistent with their job descriptions.

11. Updates the residents’ medical records at the time of transfer and discharge.

12. Initiates and updates the Resident Care Profiles and plans of care according to guidelines.

13. Participate in discharge planning. This participate includes communicating effective nursing activities to those responsible for care after discharges.

14. Executes physicians’ orders in a timely manner.

15. Disposes of infected and contaminated supplies appropriately.

16. Performs and monitors the appropriate transfer of residents, including the use of transfer equipment.

17. Assures that the nursing component of the resident record is complete.

18. Performs other duties as assigned.

KNOWLEDGE SKILLS AND ABILITIES

 Knowledge of professional nursing theory and practices

 Ability to follow oral and written directions, and to prepare and maintain records, charts, and reports.

 Ability to apply nursing techniques as directed by physicians or professional nurses.

 Ability to read, write, speak, and understand the English language.

 Ability to make independent decisions when circumstances warrant such action.

 Ability to relate information concerning a resident’s condition.

 Ability not to pose a direct threat to the health or safety of others in the work place.

 Skill in the use of the sight/hearing senses or use of prosthetics that will enable these senses to function adequately so that the requirements of the position can be fully met.

 Ability to meet the general health requirements set forth by the guidelines of the facility, which include a medical and physical examination.

 Ability to push, pull, move and/or lift a minimum of 25 pounds to a minimum height of three feet and be able to push, pull, move, and/or carry such weight a minimum distance of three feet

MINIMUM QUALIFICATIONS

Graduation from an accredited college or university with a degree in nursing.

Licensed as an LPN in accordance with the requirements of the state governing the facility.

One year of experience functioning at the LPN level.

Pay: $17.00 – $19.00 per hour
