We are seeking a Registered Nurse – Home Health Per Diem to join our team! You will be responsible for the assessment, diagnosis, and treatment of assigned patients in a home health set up. Let us know which areas you can service and we’ll match you with the right referrals!
Please note that our rates are per visit/case basis. PPE’s are provided. Documentation is quick and easy 5-10 mins long!
Responsibilities:
- Providing nurse visits to home health patients. (home visits)
- Administer nursing care to ill, injured, or disabled patients
- Diagnose and establish patient treatment plans
- Monitor and report changes in patient symptoms or behavior
- Communicate with collaborating physicians or specialists regarding patient care
- Educate patients about health maintenance and disease prevention
- Facilitate referrals to other healthcare professionals and medical facilities
- Maintain accurate patient medical records
- Provide advice and emotional support to patients and their family members
Qualifications:
- Previous experience in nursing or home health
- Ability to build rapport with patients
- Strong problem solving and critical thinking skills
- Ability to thrive in a fast-paced environment
Requirements:
- California Issued RN license
- CPR or BLS card
- Driver’s License
- Auto Insurance
- Professional Liability Insurance
- Physical Exam
- PPD/TB Test
- Immunization Record
- Proof of Covid Vaccine
- Photo (for issuance of company ID)
