Essential Duties and Responsibilities include the following. Other duties may be assigned.
Informally direct the work of the interdisciplinary team members to promote cost-effective and appropriate care and to ensure services are provided in a timely manner.
Meet with administration, nursing staff, and any other departments to ensure interdisciplinary planning of managed care services.
Continuously follow-up on the implementation of managed care resident care activities and evaluation of outcomes, along with reassessment of the residents condition as needed.
Assist with the discharge planning team and social services as needed.
Maintain current case management records.
Assist in standardizing the case management process.
Interpret managed care policies and procedures as required.
Maintain an adequate relationship with families, residents, insurance companies and hospitals.
Qualifications: Must have at least one year experience as a DON in a skilled nursing facility and an unencumbered RN license in Wisconsin.
