MDS Coordinator – Divine Rehabilitation and Nursing at St. Croix – Saint Croix Falls, WI

We are seeking an experienced Registered Nurse MDS Coordinator to join our fantastic team!

Long term care, MDS, PDPM, and CMI knowledge, as well as working knowledge of the RAI manual.

Responsibilities Include:

The MDS Coordinator’s duties include maximize case mix, documentation, review records, and work with staff.

Qualifications:

· Associates degree or greater from an accredited college

· Unencumbered RN in WI

· LTC/SNF experience preferred

· MDS experience a plus

· PDPM knowledge experience a plus

· CMI knowledge is a plus
