Work Schedule: Full Time – 80 Biweekly Hours

Shift: 8-hour, Days

Job Summary:

The Admissions Coordinator LVN is responsible for informing patients and/or clients of available services. The Admissions Coordinator LVN ensures that patients receive responsive and safe transition; coordination and continuum of care after patients are referred from hospital/Skilled Nursing Facility (SNF) to home.

Essential Functions:

  • Identifies patients who may need post-acute care.
  • Assumes initial responsibility for assessing patient/family needs for any post-acute St Joseph Health. Service including home health, hospice, infusion pharmacy and private duty.
  • Facilitates transition of patient/family to necessary services.
  • Attends appropriate meetings to promote referrals.
  • Perform EOBs for potential GIP patients and communicates information to appropriate Hospice Intake Department.
  • Assists in coordinating home health, private duty, and hospice service needs from all referral sources including but not limited to hospitals, clinics, physician’s office, skilled nursing facility and the community.
  • Ensures skilled nursing facilities understand home health, hospice, private duty, and infusion pharmacy referral process and develops tracking mechanisms to facilitate and ensure that they meet the St. Joseph Health post-acute provider network parameters.
  • Provides home care and hospice information/education at meetings with hospital, utilization review/discharge planners/case managers, skilled nursing facility, Board and Care Homes, patients, and patient’s families.
  • Obtains complete physician orders and referral information to the network intake department as needed.
  • Collects information and data regarding customer expectations, preferences and satisfaction with St Joseph Health, Home Care Services and private duty programs and services.
  • Promotes internal and external communication, problem solving, and customer need identification through regular contact with clinical area and intake department on an as needed basis.
  • Maintains comprehensive records and statistics on coordination activities.
  • Completes and adheres to annual compliance education, updates, and understands applicable policies.
  • Assist with data entry of new patient information as needed.
  • Coordinates activities with marketing team to develop strategies to increase patient referrals.

Skills:

  • Knowledge of current Medi-Cal and CMS rules and regulations.
  • Knowledge of current third-party payor requirements.
  • Ability to work with multidisciplinary team members.
  • Professional approach with all referral sources.
  • Communication skills, written, verbal, spoken with a broad spectrum of individuals.
  • Organizational skills to include prioritization, planning, implementing, and controlling.
  • Positive innovative approach to problem solving.
  • Knowledge and application of Electronic Health Records.

Minimum Position Requirements:

Education: Education to meet certification, license, or registration requirement.

Experience: One (1) year experience in health care setting.

Licenses/Certifications:

  • Active and current LVN Licensure in State of California.
  • Basic Life Support certification.
  • California Driver’s License.
  • Auto insurance.
