Patient Advocate – Gila River Health Care – Sacaton, AZ

Overview:

Assists patients with basic medical and non-medical questions or concerns regarding services performed by the enterprise. Provides multi-faceted support and serves as a resource person for information on community resources. Assists patients in accessing services for which they are eligible. Ensures that all critical tasks are performed on a timely basis.

Responsibilities:

  • Provides information, explanations, and guidance on resources to patients and their family members.
  • Coordinates with social workers to ensure that all information provided is accurate and consistent.
  • Assists patients in applying for services they are eligible for and advocates on the patients’ behalf to service organizations or agencies.
  • Routinely makes rounds in the clinic while patients are undergoing treatment to facilitate communication and identify patient needs.
  • Picks up patient prescriptions from Gila River Health Care pharmacies while patients are on dialysis and delivers as needed.
  • Communicates and coordinates with patient transport drivers to arrange transportation for patients for non-dialysis purposes such as personal errands or social events.
  • Makes visits to patients’ homes when necessary.
  • Helps administer patient satisfaction surveys while explaining the purpose and benefits of the survey.
  • Receives and responds to patient complaints while communicating these complaints to staff and suggesting solutions.
  • Attends support group meetings while serving as a resource to attendees.
  • Performs other job-related activities as requested.

Qualifications:

  • High School Diploma or GED required
  • Customer Service experience preferred.
  • Must be a member of a federally recognized tribe.
  • Ability to communicate in Pima language preferred.
  • State of AZ Driver’s License with no significant violations during the last 5 years
  • Current and continuing CPR and First Aide certifications required.
  • Demonstrates ability to relate to diverse cultures and specifically the Gila River Indian Community and/or other Native American cultures and community health services.
