NURSE PRACTITIONER/Urgent Care & Emergency Department – Janesville Main and North – Mercyhealth – Janesville, WI

JOB SUMMARY

This position provides general medical assessment, evaluation and treatment of patients referred to palliative care consultative services in the clinic, home and hospital. Position is under direction of Physician.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Assess patient data and perform interview, physical examination and begin discussing goals of care within prescribed guidelines and instructions of Physician.
  • Maintain timely consult schedule.
  • Provide patient and family education to assist patient and family to understand role of palliative care in current health situation.
  • Order, interpret, and evaluate diagnostic tests to identify and assess patient clinical problems and health care needs if required.
  • Record physical findings, and formulate plan and prognosis, based on patient condition.
  • Coordinate care with Palliative Care Physician and other health professionals; preparing comprehensive patient care plan.
  • Discuss health care plan and goals of care with Physician partner.
  • Consult with partnering physician and refer to specialized health resources for treatment and evaluation as needed.
  • Assists in developing palliative care protocols and processes.

EDUCATION & EXPERIENCE

Graduate of an accredited nursing program.

Graduate of an accredited masters of nursing program.

Meets the requirements determined by the State of Wisconsin or Illinois to perform duties defined as those of a Nurse Practitioner.

CERTIFICATION/LICENSURE

Hold valid RN license in state of practice.

Hold valid APNP license in state of practice.

Approved CPR or BLS certification required within 90 days (Other certification per unit requirements: ACLS, PALS, PEARS, NRP)

Maintain a valid driver’s license and reliable transportation with proof of vehicle insurance when requested.

Have and maintain an acceptable driving record per Mercy’s HR-01-Fleet Safety policy.

ADDITIONAL REQUIREMENTS

Passing the Driver’s License Check and/or Credit Check (for those positions requiring).

Passing the WI Caregiver Background Check and/or IL Health Care Workers Background Check.

Must be able to follow written/oral instructions.
