Handle the grievance process for all patients and residents
Visit patients daily and help interpret and address their social, psychosocial, and emotional needs.
Assist all patients with the discharge planning process and perform arrangements as necessary.
Meet with the Interdisciplinary Team to help establish a patient individualized care plan.
Nursing home experienced preferred. Document in patient charts accordingly.
Assist in planning, developing, organizing, implementing, evaluating, and directing the social service programs of this facility.
Meet with administration, medical and nursing staff, and other related departments in planning social services.
Inform the patient/resident/family of the patient’s personal and property rights.
Maintain confidentiality of all private healthcare information.
Other duties as assigned.
Requires a Bachelors college degree with certification as a LSW – Licensed Social Worker.

Pay: $20.00 – $23.00 per hour
