Registered Nurse – Nurse Navigator – Harris County – Houston, TX

Under the direct supervision of the Refugee Health Screening Program Outreach Manager, the Nurse Navigator will provide nursing, clinical case management, and other services within scope of practice to refugee patients arriving requiring medical assistance.

Duties and Responsibilities:
Case Management Coordination, Referral, and Follow-up

  • Serves as a liaison for the newly arrived refugees between the Resettlement Agencies,Harris County Public Health (HCPH) Refugee Health Screening Program (RHSP), social services, and other healthcare providers
  • Ensures all refugees who test positive for exposure to TB are referred to the appropriate referral provider
  • Receives, reviews, and prioritizes all laboratory results and ensures all abnormal laboratory values are documented in the EMR
  • Ensures abnormal laboratory values are flagged and reviewed by the RHSP mid-level providers or Program Director for referrals or treatment as indicated
  • Monitors, tracks, and follows up on abnormal test results/findings/missed appointments by telephone, postal correspondence, or home visits
  • Ensures that Notifiable Conditions are reported to the appropriate public health agency such as the City of Houston TB Program, HCPH TB and Epidemiology Programs
  • Ensures transportation and translation services are provided for the patients at their initial referral appointment

Clinical Activities

  • Assists with patient screening (i.e. drawing blood, testing urine, taking/recording vital signs, heights and weights, vision and hearing assessment)
  • Interviews and records pertinent medical/socioeconomic patient information in the EMR for the physician assessment
  • Provides patient individualized health education /counseling
  • Presents group health education sessions when indicated
  • Assists with control of patient flow through the facility during clinic visits
  • Administers/dispenses prescribed medication according to Pharmacy D regulations
  • Gives age appropriate immunizations
  • Discharges patients from the clinic following the medical providers’ care plan and instructions
  • As an integral member of the Disease Control and Medical Epidemiology Section (and Harris county as a whole), the employee collaborates and assists in other disease control and/or nursing activities from time to time as the need arises

Quality Assurance Activities

  • Evaluates and documents staff performance based on clinical chart reviews.
  • Evaluates and documents staff skills validation
  • Participates in collecting patient satisfaction surveys
  • Participates in staff meetings and case conferences, including giving oral reports on patient care activities
  • Participate in Quality Management activities related to clinical care, key performance indicators and overall program objectives


Other

  • Performs other duties as assigned, including special tasks involved in responding to an emergency event

  • Associates Degree in Nursing from an accredited school; and
  • Licensed by the State of Texas to practice as a Registered Nurse
  • Current CPR certification
  • Three (3) years of nursing experience in public health, community healthcare, and/or medical office setting
  • Experience handling complex patient load
  • Ability to work under pressure with frequent interupptions
  • Ability to file and retrive electronic records accurately
  • Ability to provide timely responses to information requests
  • Basic proficiency with Microsoft Office Suite
