Bilingual (English/Spanish) Intensive Care Coordinator – Master Level (Southbridge) – YOU, Inc. – Southbridge, MA

We are currently searching for a dedicated professional to join our team as an Intensive Care Coordinator for our Community Based Services in Southbridge, MA. Bi-lingual candidates (English/Spanish) are strongly preferred.

Responsibilities:

• The ICC provides wraparound services including care coordination, homebased assessment services, and collateral work with providers and state agencies.
• Responsible for care planning process for youth and families.
• Works as part of a team with a family partner.
• Provide clinical child welfare social work services, including conducting assessments; completing independent diagnosis of a child’s mental health status; evaluating and monitoring client service plans; assessing for risk and safety and responding to emergencies.
• Coordinate comprehensive services for youth and families, including therapeutic, educational, medical, social and other needed services.
• Collaborate with service providers and state agencies, such as DCF, while maintaining relationships with courts, hospitals and emergency and crisis service providers.

Qualifications:

• MA/MS in social work, counseling psychology, or related field required.
• Experience working with adolescents and/or latency children preferably in community based setting.
• Bilingual English/Spanish/Portuguese strongly preferred.
