Primary Responsibilities HOURS: 4PM-12:30AM. 40 Hours per Week. Flexible Evenings.
Summary:
Reporting to the
Director of Continuity of Care, the Continuity of Care Clinical Manager oversees
triage and field nurses, hospice aides, social workers and chaplains who
provide end of life care to patients and their families in all settings during
evening/night and weekend hours of operation. Responsibilities include staffing, orienting new employees;
clinical competency assessment, performance management and coordination of
visits to ensure the activities of triage and the evening/night and weekend
field team are carried out in compliance with Care Dimensions standards,
philosophies and policies
Qualifications:
- Registered Nurse, currently licensed in
Massachusetts and in good standing. - Associate’s
Degree accepted?; BSN preferred. - Minimum
three years of nursing experience in a home health, oncology or hospice
setting. - Previous
supervisory experience preferred. - Ability
to apply and adapt the basic principles of nursing to various situations
and to assist with the nursing care of patients according to the
interdisciplinary care plan. - Ability
to supervise, guide and develop the skills of professional and
non-professional clinical staff. - Ability
to exercise independent judgment. - Excellent
verbal and written communications skills with individuals and groups. - Demonstrated
ability to assess and respond to the needs of families and referral
sources in a professional and knowledgeable manner. - Ability
to visit patients in their place of residence ( home, hospital, nursing
home) to provide service, supervise and evaluate clinical staff or assist
in an urgent or emergency situation. - A
licensed driver with an automobile that is insured in accordance with
state and/or organization requirements and in good working order.
Performance Standards: Proficient in responsibilities specific to
the position.
- Coordinates
the activities of the field staff, including nurses, home health aides,
social workers, chaplains and contracted staff in response to patient and
family needs. - Makes
patient care assignments, taking into account care requirements of
patients and clinical expertise of staff members. - Assesses
clinical staffing patterns and needs and consults with the Director of
Continuity of Care to develop effective staffing pattern. - Assists
nursing staff with requests for services, response to emergencies, and
communications with physicians, families and referral sources. - Effectively
collaborates with the Referral Center to ensure appropriate, timely
response to referrals from the community, nursing homes and physicians.
