Registered Nurse (RN) – Connected Home Health & Hospice – Portland, OR

Job Opening: HOME HEALTH RN (REGISTERED NURSE)

• Primarily East-side Territory of Portland Metro including Sandy, Gresham and Troutdale
• Preferred Full-Time at 32-40 hours per week but will consider Part-Time at 30-31 hours per week with M-F availability
• 15 – 23 productivity-point expectations per week (average of 2 – 4 visits per day)
• No regular After-Hours/On-Call responsibilities

RN Essential Duties

· Provide one-on-one skilled nursing care and treatment to patients in their home

· Development and implementation of Plan of Care

· Provide Skilled observation, assessment and evaluation

· Patient and family/caregiver support and education

· Cohesive and collaborative care with LPN and Therapy Team

Requirements and Qualifications

· Registered Nurse (RN) licensure in Oregon

· At least 2+ Years RN experience is required

· Ability to work independently and demonstrate critical thinking

· Current BLS certification from AHA

· Be a licensed driver with an insured automobile in good working order

· Ability to comply with the State COVID Vaccination Mandate
