Clinical Case Manager Position – Eastern Nassau County
Agency: Visiting Counselors of New York, LCSW, PLLC (VCNY)
Clinical Case Manager (LMSW or LCSW required)
Job Description:
60% – Clinical Services
- Manage a clinical caseload of 20-25 clients.
- Provide individual counseling services to older adults.
- Complete clinical progress notes, consultation notes, contact notes, treatment plans and any other clinical documentation in a timely manner.
40% – Case Management Services
- Assist clients in locating and applying for housing, entitlements, and various resources.
- Connect clients with medical and other community providers.
- Follow-up with new referrals and referral sources.
- Coordinate with other team clinicians to address case management needs of clients.
Requirements:
- Hold an active NYS license as a LMSW or LCSW.
- Have independent and reliable transportation and a valid NYS driver’s license.
- Willingness to meet with clients in their home throughout Nassau County.
- Maintain personal liability insurance.
- Experience working with older adults and/or adults with neurodevelopmental disorders.
- Experience providing individual counseling services.
- Experience assisting individuals with applying for various benefits.
- Knowledge of local resources and entitlements.
- Ability to work with other practitioners to help meet client needs.
- Be responsible for an agency phone, if required
Salary& Hours:
- Hours: 37.5 hour minimum
- Salary: $58,000 to $65,000 yearly
