POSITION TITLE: Mental Health RN Case Manager

REPORTS TO (TITLE): Mental Health Supervisor


JOB SUMMARY:
Under the general supervision of Mental Health Supervisor, this position is responsible for psychiatric case management including direct patient care and education around mental health and social service needs; monitoring and mental health referrals; managing prescription refill requests, prescribed by mental health providers.

The case load will be drawn from Lombardi, Vista, and Brookwood Health Psychiatry services.


SPECIFIC TASKS/DUTIES:

  • Work as a key member of the care team for complex patients who are part of the psychiatry services at Santa Rosa Community Health in the role of a nurse case manager.
  • Work closely with psychiatry team members, including other Mental Health Nurse Case Managers and Social Work Care Manager, to coordinate care for patients transferring to and from Sonoma County Behavioral Health, hospitals, and other psychiatric facilities.
  • Work closely with the SRCHs Mental Health and psychiatry teams in case consultation and discussion for a defined set of patients.
  • Serve as back-up during absences for other Mental Health Nurse Case Managers.
  • Support Joint Commission Readiness to support quality and safety for patients.
  • Under direction and with standing orders, use independent judgment within the licensed capacity to meet as needed with individual clients, on site, in other locations, or by telephone.
  • Assist mental health staff in coordinating mental health referrals to outside community resources for patients in the mental health department.
  • Monitor and track referrals to inside programs, including other mental health services.
  • Educate patients about community based mental health recovery programs.
  • Perform telephone callbacks, prescription refills, ordering labs, etc. as assigned by SRCH mental health providers.
  • Follow all established clinic protocols for patient care and clinical duties and assist with quality assurance functions.
  • Participate in triage of patient needs.
  • Contact patients with abnormal labs or tests.
  • Provide ongoing medical management of SRCH Clozaril patients.
  • Assist with coordination of patient transportation for referral services when needed.
  • Provide one-on-one and/or telephone case management for mental health patients in need of community resources.
  • Act as a resource/contact person to clinic staff.
  • Perform input and analysis of data related to care of patients receiving mental health services.
  • Assist with chart audits and other means of documentation of on-site grant programs.
  • Participate in ongoing training.
  • Other duties as assigned.

REQUIRED QUALIFICATIONS:

  • Registered Nurse (RN) with current, active license.
  • Minimum of one-year experience with direct patient care.
  • Experience working with chronically mentally ill patients preferred.


SKILLS AND KNOWLEDGE:

  • Bilingual Spanish and English strongly preferred.
  • Familiarity with general community resources for patients in Sonoma County.
  • Knowledge of principles of nurse case management for the patient with chronic disease.
  • Knowledge of electronic medical records and tracking systems.
  • Strong written and verbal communication skills and interviewing skills, including charting/documentation.
  • Demonstrates knowledge of compliance issues within the community clinic environment.
  • Demonstrates willingness to report any incident that is unusual or incompatible with accepted clinic procedures.
  • Maintains confidentiality of patients at all times.
  • Strong interpersonal skills. Ability to interact sensitively with persons of various social, cultural, economic and educational backgrounds.
  • Proficiency with Microsoft Office applications including Outlook, Word, Excel and PowerPoint.
  • Strong organizational skills with ability to prioritize projects, work independently and as part of a team, manage multiple tasks, and meet deadlines.
  • Good judgment, problem solving and decision-making skills.


PHYSICAL REQUIREMENTS:

While performing the duties of this job, this position is frequently required to do the following:

  • Use standard office equipment and access, input, and retrieve information from a computer. Use computer keyboard with manual and finger dexterity and wrist-finger speed sufficient to perform repetitive actions efficiently for extended periods of time.
