The Manager of Case Management (MCM) reports to the Director of Housing Stability and is responsible for overseeing the case plans, after care, employment, and rehousing efforts within The Crossing. The MCM will work in close cooperation with the Homelessness Resolution Team, the Clinical Director, Community Life Director and operations to implement strategies to impact homeless resolution through client centered interventions.

The Manager of Case Management is responsible for the day-to-day oversight of the Case Managers services, clients and staff, bringing items of concern to the attention of the Director of Housing Stability.

Hiring Range: $47,800 – $59,800 annually, based on qualifications and experience.

RESPONSIBILITIES

  • Ensure that Case Managers maintains the case management and supportive systems necessary to support movement out of homelessness and into available, appropriate, and affordable housing or other long-term solutions, with long-term stability as the goal.
  • Ensure that the Case Managers are facilitating movement of clients towards core matrix, updating appropriate assessments and case notes
  • Provide oversight and training to case management team ensuring that they remain housing-stability-focused, and client centered with the appropriate level of client-based needs addressed within the rehousing system.
  • Ensure that all case managers are entering the appropriate data into HMIS and SCRIBE regarding the individualized housing plans and assessments to facilitate outcome reports to both internal and external partners.
  • Work with DRM data specialist to develop processes and prompts within SCRIBE to facilitate proper recording and coordinate efforts toward HMIS training and reporting.
  • Engage in the facilitation of care teams including counselors, including case managers, front desk, program coordinators, work readiness supervisors and chaplains, for individuals accessing long-term programs at The Crossing
  • In residential environments serve as the coordinator between operations, Community Life, Clinical and Homeless Resolution Teams to facilitate efforts

. Requirements:

  • Bachelor’s degree or higher-level degree in a Human Services field or the equivalent required, Master’s degree preferred.
  • 1+ years of experience leading teams,
  • 3+ years of direct engagement with individuals experiencing homelessness and identifying barriers to housing.
  • Documented experience case management teams
  • Documented experience with staff development and evaluation
  • Proven ability to maintain composure during stressful situations
  • Well-developed interpersonal and relationship building skills
  • Clear communication skills – both verbal and written
  • Proven ability to manage conflict with and between, subordinates, peers, and superiors, expeditiously and respectfully.
  • Track-record of work with low-income, homeless and/or indigent individuals and families
