Coordinates the total nursing care for patients through administration of the case management. Provides leadership by working cooperatively with ancillary departments and other patient team personnel in maintaining standards for professional nursing practice in the clinical setting. Oversees staffing, training, and is a liaison with physicians, hospital personnel and departments. Is responsible for the utilization of advanced modalities and hyperbaric medicine through compliant administration of case management and utilization review, Performance Improvement initiatives, chart reviews and audits.

Drives advanced modalities through rigorous case management, coordination, productivity, and effective communication of care and demonstrates leadership skills to enhance collaboration is is responsible to assess and evaluate staffing based on patient care needs
Responsible for working with Hyperbaric Safety Director and Institute Director to identify and assign staff appropriately to meet the daily patient load
Coordinates, assigns, and validates the care provided by clinical team members.
Manages, trains and assists staff in providing quality patient care.
Develops performance standards and integrates quality improvement activities for the department
Works collaboratively with the Institute Director, develops and implements education programs for physicians and staff regarding wound care
Provides Institute Director with required operational reports and assists with updating and maintaining employee files for Wound Care staff
Provides patient care updates as appropriate to Institute and Medical Director
Demonstrated clinical competence in the area of assignment and serves as a resource /preceptor new employees
Facilitates and maintains lines of communication with the various ancillary departments utilized by the Institute
Drives team conferences using case management report for the development of individualized nursing care plans
Identifies and establishes Performance Improvement Initiatives, as well as chart audits
Establishes and maintains communications with patients and other significant individuals; utilizing teaching opportunities
Provides case management functions for patients as directed and concurrent documentation of patient’s care in the case management report

Participates with committees as requested, contributes on a regular basis in the identification of opportunities for process improvement.
Participates in new staff orientation as appropriate; may serve as preceptor, providing guidance and development of skills for Wound Care Technicians
Maintains the standards of nursing care and implements the policies and procedures of Innovative Healing Systems and their partner hospital
Participates in team conferences in development of individualized nursing care plans as needed and attends and participates in and provides staff meetings and required in-services
Possesses strong verbal communication skills; demonstrating ability to communicate with adult and geriatric age groups.
Ability to work independently.and collaboratively in team environment.
Strong leadership and team building skills and the ability to adapt to changing patient flow schedule and to communicate and work closely with physicians.
Possesses knowledge of EMR system(s) and documentation requirements.and knowledge of patient confidentiality standard and possesses knowledge of outpatient provider services.
Associate’s or Bachelor’s degree in Nursing.
Minimum 2 years of nursing experience in wound care/treatment environment.
Minimum 2 year of nursing/staff leadership experience in an outpatient environment.
State issued license as Registered Nurse in good standing.
ACLS/BLS issued from American Heart Association

Job Type: Full-time

Pay: $70,000.00 – $85,000.00 per year
