ShareSTAFF HealthCARE is in search of a RN Case Manager who leads with empathy and embodies compassion to make a difference in the lives of the patients they serve.
This is a contract assignment Monday thru Friday 8am-5pm
Job Summary
Provide nursing assessment, planning and care to maximize the comfort and health of patients and families in accordance with the interdisciplinary plan of care.
Essential Job Functions
- Receive patient assignments from the Director of Clinical Operations.
- Assume responsibility for a patient/family that includes assessing, planning, implementing, and evaluating care.
- Initiate communication with attending physicians, other hospice staff members and other agencies as needed to coordinate optimal care and use of resources for the patient/family.
- Maintain regular communication with the Manager, Clinical Operations to review care.
- Maintain regular communication with the attending physician concerning patient/family.
- Obtain data on physical, psychological, social, and spiritual factors that may influence patient/family health status and incorporate that data into the plan of care.
- Maintain up-to-date patient records so that problems, plans, actions and goals are accurately and clearly stated, and changes are reflected as they occur.
- Accept responsibility for coordinating the physical care of the patient by teaching primary caregivers, volunteers, employed caregivers, or by providing direct care as appropriate.
- Inform the manager, Clinical Operations of unusual or potentially problematic patient/family issues.
- Attend the Hospice Team Meetings and other patient conferences as deemed necessary by the Manager and Clinical Operations.
- Provide appropriate support at time of death.
Registered Nurse Case Manager Requirements
- Current Licensure
- Updated CPR/BLS
- COVID Vaccine
- Case Management Experience Preferred
- Home Health and Hospice Experience Preferred
Job Types: Full-time, Contract
Pay: $40.00 – $45.00 per hour
