Behavioral Health Case Manager – River Region Human Services Inc – United States

This position provides behavioral health case management services to clients. Supporting them and acting as an advocate for the client.

  • Work with clients with mental health or substance use disorders to help link services.
  • Professionally develop a knowledge-based, comprehensive case management system of service enabling the client to function at an optimal level in the home and community.
  • Ensure on-going quality assurance/utilization and compliance with the agency’s policies and procedures and Medicaid standards.
  • Ensure the confidentiality of all client records.
  • Involve the client and family in planning to appropriate responsibility. Initiate compliance and follow through on participation in the treatment programs and service plans.
  • Develop an appropriate service plan within 30 days of enrollment.
  • Conduct regular monthly visits with each client (at a minimum 32 Billable Hours per week) and provide in-home crisis intervention in the event the client is at risk of hospitalization.
  • Involving the client and family’s progress, including a written case progress report from the involved providers and/or residential facility in accordance with established procedures.
  • Complete accurate documentation, reports, service plans, and activity reports in a timely manner as is outlined in the case management general procedures and by their due dates.
  • Assist in formulating and arranging appropriate discharge services/treatment, residential, educational, medical, legal, and social services for client in the caseload.
  • Assess client needs in their environment or the office and connect to appropriate resources.
  • Work with client to formulate and execute individualized service plans.
  • Monitor delivery of services on a continuous basis.
  • Complete Medicaid billing and documentation within the required timeframe.
  • Complete chart audits to ensure compliance and quality of services provided.
  • Complete annual required trainings.
  • Other duties as assigned.

  • Candidates will have graduated from an accredited university with a minimum of a Bachelor’s degree in Psychology, Social Work, Marriage and Family Therapy, Mental Health Counseling or related Social Services or Behavioral Health field.
  • Candidates must also possess at least 1 year of experience working with adults with substance abuse and mental health disorders.
  • Acceptable professional certification as an addictions and/or criminal justice professional preferred.
  • Must have knowledge and comply with state and federal statutes which affect the target population.
  • A valid Florida driver’s license and a good driving record are required.

  • Must have the ability to perform case management tasks.
  • Must be able to provide own transportation.
  • Must be eligible for Level 2 DCF Background Screening approval.
  • Must be able to communicate effectively verbally and in writing. Computer competency required.
  • Must possess excellent organizational skills.
  • Must have the ability to handle confidential information appropriately.
  • Ability to interact with people from diverse cultural, economic and ethnic backgrounds.
  • Ability to set appropriate boundaries and limits with clients.
  • The ability to correctly document and complete services in the RRHS Electronic Health Record according to the standard RRHS timelines of documentation.
