Registered Nurse – Supreme Management and Associates, LLC – Blacksburg, SC

Register Nurse (RN) must meet the following requirements:

  • Currently licensed by the South Carolina State Board of Nursing.
  • Able to assume responsibility for in-service training for aides by group meetings, individual instruction or workshops.
  • Capable of evaluating the aide’s competency in terms of his or her ability to carry out assigned duties and their ability to relate to the participant.
  • Must have had training and/or background on the complex treatment issues regarding the care of the head and spinal cord injured.
  • Visit client’s home prior to services being provided by the aide. Record the visit in the care call system from the client’s home and documented in the record.
  • Review the care plan/service plan and develop a task list for the aide.
  • Give the client written information regarding advanced directives and inform him/her of their rights. The nurse supervisor(s) is required to sign and date all required forms and the client is required to also sign and date a statement that he/she has received this information.
  • Nurse supervisor(s) can’t discuss services provided and must refer that client back to their case manager/service coordinator for additional information.
  • Supervising nurse must be accessible by phone during any hours services are being provided.
  • Supervising nurse must provide and document supervision of, training for, and evaluation of aides.
  • Supervising nurse must make a supervisory visit to the client’s home/place of residence within thirty (30) days after services are initiated.
  • Supervising nurse must visit the client’s home/place of residence at least once every four (4) months. The aide must be present during at least one (1) of the supervisory visits during the each twelve (12) month period.

The Nurse Supervisor’s report of the on-site visit(s) must include, at a minimum:

  • Documentation that the client’s needs are being met;
  • Documentation that services are being delivered and are consistent with the individual’s care plan/service plan;
  • Reference any issues or complaints with the client or family member/responsible party has been lodged/made;
  • A brief statement regarding any changes in the client’s service needs; and, supervisor’s original signature and date is required.
  • Assist aides as necessary as they provide approved services as outlined in the service plan/care plan. Any supervisory visits must be documented in the client’s record within thirty (30) days of the date of visit.

Pay: $16.00 – $25.00 per hour
