Licensed Practical Nurse

The Licensed Practical Nurse will deliver care in collaboration with the Registered Nurse. Working under the immediate supervision of Registered Nurse and Director of Nursing, the Licensed Practical Nurse will record vital signs, administer injections and assist with procedures and documentation.


Education Requirements:

  • High school diploma or equivalent
  • Practical Nurse License (LPN) in the State of Wisconsin


Experience:

  • One (1) year of technical training; supplemented with one (1) year of related work experience.


Essential Functions:

  • Assume self-responsibility for all direct patient care activities as indicated by the Registered Nurse within the scope of the Nurse Practice Act and policies and procedures.
  • Give direct resident care according to the patient care plan and individual patient needs.
  • Provide appropriate information to assist the Registered Nurse in the preparation and maintenance of an effective resident care plan.
  • Keep a detailed care report of all residents under care.
  • Reinforce instructions given to the resident by the physician and Registered Nurse and refer new needs for instruction to the Registered Nurse.
  • Administer prescribed medications to resident under exact instruction by the physician following the facilities medication policies and procedures.
  • Perform selected prescribed treatment and nursing measures for the resident under specific guidelines.
  • Notify the Registered Nurse of significant findings or changes in the residents’ physical and/or mental condition.
  • Initiate C.P.R. procedures when there is a sudden cessation of residents heartbeat or respiration’s.
  • Initiate emergency assistance as per the facilities policies and procedures.
  • Act as a role model for ancillary personnel.
  • Maintain the confidentiality of the residents’ clinical records.
  • Practice within the limits of the Nurse Practice Act and understand legal consequences of nursing action.
  • Involve patient and/or family in care of patient as appropriate.
  • Keep skills up to date to increase knowledge and productivity.
  • Seek educational opportunities to increase clinical competency.
  • Prepare and record clinical notes for the clinical records.
  • Other duties as assigned.
