Salary: Pay Per Visit Model, Estimated Annual Earnings: $77K, plus a $15,000 First Year Success Bonus (paid out quarterly)

Job Description:

As a part of our team, you will be responsible for providing high-quality, patient-centered home health care to patients in the comfort of their own homes. This role offers flexible schedules, requires minimal weekend shifts, and pays per visit. Successful candidates will receive a comprehensive 6-week RN onboarding program, a rewarding compensation package, and the potential to partake in a company vehicle program.

Primary Duties and Responsibilities:

  • Provide high-quality home health care to patients in their homes
  • Monitor patient conditions and adjust care plans as needed
  • Collaborate with the interdisciplinary healthcare team to ensure the highest quality of care
  • Maintain clear and accurate documentation of care provided
  • Work with schedulers to assist in the creation of monthly care plans

Required Skills, Qualifications, and Experience:

  • Associate degree or higher in Nursing
  • Strong communication and interpersonal skills
  • Compassionate and caring demeanor

 

Preferred Skills, Qualifications, and Experience:

  • At least one year of professional experience as an RN, with home health experience preferred
  • Ability to travel as required

Required Licenses, Training, and Certifications:

  • Valid Registered Nurse (RN) license/certification in Pennsylvania
  • Valid American Heart Association Basic Life Support (AHA BLS)
  • Valid American Heart Association Advanced Cardiac Life Support (AHA ACLS)
  • Valid driver’s license, reliable mode of transportation, and auto insurance
