Part Time Physical Therapist Assistant – Illinois Center for Rehabilitation & Education – Roosevelt (ICRE-R) Illinois licensed (mandatory) Physical Therapist or Physical Therpaist Assistant needed for residential school located in the Medical District on the west side of Chicago for students ages 18-22 with severe physical disabilities and other health impairments (such as spina bifida, muscular dystrophy, cerebral palsy, etc.). The current number of students is 32-35.
Evaluations / Treatment / Therapy Groups
- Training: provide in-service training to students, staff, parents/guardians regarding student’s individualized treatment, transfer, exercise programs, etc.
- Provide new student’s room accessibility assessment for program
- Provide transfer training for new students to residential services and other staff
Physical Therapy: provide individualized Physical Therapy to ICRE-R transition students (assess, evaluate, and re-evaluate students as needed)
- Design/provide and implement individual physical therapy treatment goals for ICRE-R transition students
- Provide approximately 10-12 students per day or 35 students per week with physical therapy services
- Monitor and document student progress
- Complete initial evaluations, re-evaluations, assessments, and exit summaries for students
- Complete monthly billing and documentation
- Provide community resources for PT services as needed
Therapeutic Modules: provide therapeutic modules/groups for transition students with therapeutic service staff – create and design transition curriculum modules which include: transfers, exercise programs, mobility lessons, etc.
- Provide individualized therapeutic module goal sheets for students
- Provide individualized physical therapy goal sheets for students
- Monitor and document student progress and goals
Orthotic Clinic: follow-up on all medical referral pertaining to orthotics
- Coordinate orthotics clinics with outside vendors for ICRE-R students (four times per year)
- Provide referral sources for PT services within the community
Office Work / Meetings
- Maintain ICRE-R Physical Therapy files and medical files
- File student reports and documents, physician referral forms (PRF), complete monthly physical therapy IDPA Medicaid billing
- Provide accurate work timesheets and follow all ICRE-R timekeeping rules
- Provide necessary release and consent forms for PT and AT for nursing/medical services
- Request maintenance and service orders for physical therapy areas to the Business Office Administrator (work requests and work orders)
- Manage equipment for physical therapy
- Report and request maintenance for physical therapy
- Order and maintain equipment and supplies for
- Office duties include: ordering stock items, copying and distribution of information, shred documents, answer telephone calls, pick-up mail, complete correspondence
- Complete ICRE-R inventory for physical and aquatic therapy
The Physical Therapist Assistant will work an estimated 16 hours per week, preferably on Tuesdays and Thursdays from 8:30AM to 5:00PM with a 30 minute lunch.
The facility operates on a school calendar. The facility will be closed and paid time off will be given for holidays, two weeks off for Christmas, one week for Spring break, and six weeks off in the summer. Please note there is no health insurance provided.
