Care Coordinator (preferably Spanish-speaking) – Drew & Associates – Washington, DC

Join a developing department to ensure the highest quality behavioral health outcomes by evaluating client needs, entering information into our system and make linkages to services. Candidates may have some telework opportunities; however, working in the community and in the office is required. Optimal candidates will have:

– Knowledge of health/behavioral health/substance use services;

– Understanding of tracking and data;

– Experience with technology including health/behavioral health record keeping;

– Comfort working with a diverse team in a changing industry;

– Flexibility to work evenings;

– Comfort visiting people’s homes;

– Providing individual coaching and support to clients and staff.

REQUIREMENTS:

– Reliable and consistent with utmost integrity;

– Drivers license with good record;

– 4-year degree in health, social and human services.

PLUSes for increased compensation:

– Experience with Data Witts (DC-DBH’s substance abuse system);

– Spanish fluency (reading, writing, speaking).

Job Type: Full-time

Pay: $21.00 – $27.00 per hour
