DIET-001: Dietician – Per Diem

Job Summary:

The Licensed Registered Dietitian will be available to evaluate the nutritional needs, provide nutritional counseling and create a clinical nutrition treatment plan for hospice and home health patients. The LRD will counsel hospice patients and families who experience challenges and conflict with end of life care dietary issues and home health patients with dietary issues. The patient population can include newborns through geriatric patients. The Dietitian is responsible for providing dietary/nutritional consultation to the clinical staff in accordance with the interdisciplinary plan of care, appropriate standards, policies and procedures.

Primary Job Duties:

  • Provides appropriate dietary counseling as requested by the interdisciplinary team to the hospice and home health patient and family regarding dietary issues.
  • Responsible for comprehensive nutrition assessment for each referred patient within a timely manner. Reviews lab work and intervenes as appropriate in collaboration with the patient’s attending physician and the hospice / home health interdisciplinary team.
  • Recommends appropriate feeding methods/formula or meal plans to meet the patients’ symptomatic and palliative nutritional needs and increase the patient’s level of comfort.
  • Appropriately documents and communicates all required information in accordance with agency policies and procedures, accreditation, regulation, and legal requirements.
  • Annually continues personal development to enhance practice and profession.
  • Organizes and performs work effectively and efficiently.
  • Maintains and adjusts schedule to enhance program performance.
  • Demonstrates positive interpersonal relations in dealing with all members of the team.
  • Maintains and promotes customer satisfaction.
  • Effectively demonstrates the mission, vision, and values of Lower Cape Fear LifeCare on a daily basis.

JOB SPECIFICATIONS

  • Education: Bachelor’s degree in nutrition from an accredited (will be exempt) or comparable verifiable training/experience (will be nonexempt) required.
  • Licensure / Certifications: Current state licensure. Preferred membership to the Academy of Nutrition and Dietetics.
  • Experience: Minimum of one year of experience in a health care setting. Experience in working with clients who have dietary problems related to illness. Previous work with terminally ill patients/families preferred.
  • Essential Technical / Motor Skills: Ability to speak clearly to communicate with patients, families, physicians and staff regarding patient’s condition and care. Must be literate in the English language.
  • Interpersonal Skills: Ability to develop positive interaction with patients, families, physicians and staff to effectively care for the patients.
  • Essential Physical Requirements: Ability to lift and/or maneuver objects weighing at least 45 pounds or less in the assessment and implementation of patient care. Requires pushing, moving, lifting of patients.
  • Essential Mental Abilities: Must be able to assess a patient’s condition, formulate a Plan of Care, implement appropriate interventions to meet patient/family outcomes. Requires higher level of mental faculties accompanied by short and long-term memory.
  • Exposure to Hazards: Frequent exposure to blood and body fluids, infectious diseases. May be exposed to dangerous animals and traffic hazards while home visiting. May encounter patients and other situations that present a potential threat to personal safety. May encounter temperature changes and weather extremes.
  • Other – Hours of Work: Flexible generally Monday through Friday. Also required to attend some early morning and after hours meetings. Due to nature of work, hours can be long and irregular.
  • Driving: May be required to drive. Must have a valid driver’s license, proof of insurance, and reliable transportation. Will be required to sign a release authorizing HR to check DMV records every three years. Will be subject to the Safe Driver Policy.
  • Population Served: Patient populations from infants through geriatric with life-limiting illness.
