PACE is an innovative program designed to provide and integrate all acute, primary, preventive and supportive health services to high-risk individuals living in the community. Our goal is to keep our members living safely in their homes for as long as possible. We provide services in the community and at our PACE center which is open 8 AM to 5 PM Monday – Friday.

This position anticipates and supports the needs of our members through ongoing assessment, planning, coordination, real-time intervention and follow-up. This nurse requires strong critical thinking and problem solving skills with a focus on preventing unnecessary emergency, hospital and skilled nursing utilization.

In addition to managing the day-to-day functions of the PACE clinic, this position works closely with our physician and other team members to effectively monitor, identify and address any potential risks to the health and wellness of those we serve. This position will also be responsible for providing the highest level of customer service and compassion to our members and their families.

This nurse also maintains a close partnership with our members, their families and the PACE Team. The Home Care Nurse has the responsibility and autonomy to prioritize their schedule to effectively monitor, identify and address any potential risks to the health and wellness of those we serve. This position will also be responsible, in collaboration with the PACE Team, for facilitating advance care planning and the provision of quality care and services through end-of-life.

The Home Care & Clinic Nurse is an integral part of our interdisciplinary team. Within this team, this nurse will participate through open communication, active listening, collaboration and multi-disciplinary planning. Successful candidates will exhibit strong clinical, home care, and leadership skills.

  • Clinic Nurses;
  • Home Care Nurses;
  • Physical Therapist;
  • Occupational Therapist;
  • Licensed Masters Social Work;
  • Recreational Therapist;
  • Dietitian & Diabetes Educator;
  • Transportation Director/Drivers; and
  • Personal Care Aides
  • RN degree from an accredited school of nursing or college/university
  • Homecare experience preferred
  • Strong critical thinking and problem solving skills desired
