This position is working within the Criminal Justice System for Cheatham, Robertson and Sumner Counties

SCHEDULE:

Monday-Friday, 8:00am-4:30pm

Position responsibilities:
1. Proactively identify and engage consumers who are seriously and persistently mentally ill (SPMI) in the Criminal Justice System.
2. Identify service providers in various organizations, institutions and agencies serving or having contract with SPMI consumers.
3. Develop community resources and coordinate existing resources for this population.
4. Meet with identified service providers to educate them about the Agency’s programs and to solicit their assistance in identifying consumers who would benefit from the program.
5. Assess referrals to determine their appropriateness for mental health and/or other services and to assess their immediate needs.
6. Develop and coordinate plans for release from jail.
7. Link current consumers of mental health services with their service providers.
8. Conduct follow-up activities with consumers when released from jail into the community programs and services to ensure successful integration into the community from incarceration.
9. Complete all paperwork necessary, including imputing information into the TNWITS system.
10. Present assessment and enrollment paperwork to appropriate staff members for assignment.
11. Provide consultation to Care Managers and other mental health providers regarding assessment evaluations, as needed.
12. Inform program personnel and/or community of trends, issues and concerns.
13. In conjunction with the TDMHSAS staff, develop a training format and curriculum for the purposes of providing mental health training activities to law enforcement officers, correctional personnel, transportation agents and other appropriate Criminal Justice personnel.
14. Ensure program protocols/policies and procedures are carried out.
15. Submit monthly and quarterly reports to TDMHSAS as indicated in grant requirements.
16. Promptly address team concerns and work to obtain resolution.
17. Provide support and consultation for Care Managers with consumers in crisis situations.
18. Review clinical documentation to ensure this documentation is individualized and meets consumer’s needs.
19. Ensure consumers are provided efficient transitions within the Agency and in the community.
20. Assist Care Managers and other staff with referral and linkage to community resources and advocate for consumers when necessary.

Essential skills and experience:
Master’s degree (M.A., M.S.) in a health-related field of counseling, psychology, social work or sociology is preferred.
Master’s degree in other related fields, such as Criminal Justice, will be considered.
Internal applicants must have minimum 2 years Care Management experience.
