Fedelta Home Care is a leading in-home health care agency that has an exciting opportunity for a Nurse Care Manager to join our team! This position is responsible for conducting assessments and updating plans of care for our skilled nursing clients. You will promote quality care and a safe and therapeutic environment in home. This requires regular local travel between assigned client residential locations serving King, Pierce and/or Snohomish counties.

Essential Functions:

  • Applies nursing assessment skills and judgment open new Home Health cases and maintain an existing case panel of Home Health nursing clients with the goal of creating a safe and therapeutic residential environment that promotes clients’ safe and quality care in their home and in the community beginning on the day of admission.
  • Implement individualized Home Health Plans of Care (POCs) as assigned based on physician orders, nursing needs assessment and client input. Create client’s initial chart forms and evaluate the effectiveness of POCs on an ongoing basis. Work with client and their prescribing providers to revise as necessary based on appropriate goals and evaluation outcomes.
  • Ensure client’s physicians and other appropriate health care providers are advised of client’s response to care and any significant change in client’s condition or needs, as necessary.
  • Conduct a thorough Beside Orientation (BSO) for each new attending nurse that has been assigned to a client on the Nurse Care Manager’s caseload.
  • Conduct visits with each attending nurse assigned to the Nurse Care Manager’s caseload a minimum of once per month. Address questions or concerns related to on-going patient care, interpersonal dynamics in the home environment, scheduling or aspects related to employment with Fedelta.
  • Perform monthly chart audits to ensure timely and accurate implementation of prescriber’s medical orders and professional documentation of care.
  • Assist Director of Clinical Services with provide mentorship and supervision of our licensed nurses. Perform annual skills assessments, performance evaluations, and coaching of licensed nurses as directed.

Qualifications:

  • Current Washington State LPN or RN license in good standing, CPR/First Aid or BLS certification.
  • At least 2 years’ experience in pediatrics, tracheostomy and ventilator care, oxygen therapy, end-of-life and/or other specialties in a home health setting.
  • Prior experience conducting skilled assessments such as medical history, physical health, mental condition, and cognitive screening.
  • Previous supervisory experience of licensed nurses or other healthcare staff is highly desired.
  • Knowledge of Washington State Home Health regulations (WAC-246-335) is highly desired.
  • Valid Driver’s License, satisfactory driver abstract, proof of auto insurance, and dependable automobile.

Salary and Compensation!

  • This is an exempt position
  • Salary is dependent on experience, beginning at $70,000+
