No Place Like Home Health is a veteran-owned Skilled Home Care Agency located in Livonia, Michigan with a position in Genesee County
This position is paid per visit.
THIS POSITION IS PART TIME TO START, AND FULL-TIME OPPORTUNITIES IN THE FUTURE AND WILL BE WORKING WITH OUR NATION’S VETERANS!
We are currently looking for experienced License Practical Nurse (LPN) to join our team! You MUST have a minimum of 1 year of Skilled Home Care and OASIS Visit experience.
Please Note: We are unable to consider any candidates who fail to meet this minimum requirement.
RESPONSIBILITIES:
- Conduct an initial and ongoing comprehensive assessment of the patient’s needs, including Outcome and Assessment Information Set (OASIS) assessments at appropriate time points;
- Obtain a medical history from the patient;
- Evaluate the patient and home situation to determine what health teaching will be required, if additional services are needed and the frequency of this service;
- Develop and implement the nursing care plan;
- Document services and education provided to the patient, using an Electronic Medical Record (EMR) application; No Place Like Home Health. MUST be able to adhere to strict deadlines for Start of Care (SOC), revisits, re-certifications and discharges;
- Coordinate total patient care
- Participate in Case Conferences, In-Services and mandatory Staff Meetings as needed.
MINIMUM REQUIREMENTS:
- Must have a valid, unencumbered License Practical Nurse (LPN) from the State of Michigan;
- Must have a minimum of 1 year of Skilled Home Care / Visit nursing experience;
- Must have a valid Drivers License, your own reliable transportation and proof of current Auto Insurance;
- Must be willing to travel within a designated radius (from your home address); this radius will be agreed upon during the interview process and upon hire.
Job Types: Part-time, PRN
Pay: $45.00 per hour
