Sign-on bonus of up to $5,000.00

Available positions in and around Longview, WA:

  • Hospice Case Manager M- F 8a-5p.
  • Home Health Case Manager M-F 8a-5p.
  • Weekend Revisit/Admit Hospice Saturdays/Sundays.
  • Hospice Admit RN 32 to 40 hours a week.
  • NOC shift Care Center 6p – 6:30a 3 days a week.


SUMMARY:

Primary functions include planning, providing, and evaluating skilled nursing care to patients and families in their homes. The ideal candidate will evaluate and coordinates the needed course of care. Confers and collaborates with the physician and other interdisciplinary team members in implementing the care plan.

DUTIES AND RESPONSIBILITIES:

  • Makes the initial visit, to evaluate the patient and family needs. Develops and implements nursing treatment plans for all patients referred to him/her, based upon data from the nursing history patient and family and assessment and the physician’s plan of treatment.
  • Provides services requiring substantial and specialized nursing skills. Initiates appropriate preventive and rehabilitative nursing procedures, coordinating a plan of treatment with physical, speech and occupational therapists, medical social workers, and home care aides as necessary.
  • Regularly re-evaluates the patient and family needs, and revises the plan of care as appropriate.
  • Plans for and participates in case conferences with the interdisciplinary teams.
  • Reports significant changes in patient’s condition, signs, symptoms, and needs to the physician and other agency personnel. Consults with the department coordinators and medical staff.
  • Prepares concise legible clinical notes, progress notes, summary reports, re-certifications, and discharge summaries completely and in a timely manner.
  • Is responsible for tasks delegated to the Care Aide and the manner in which they are accomplished. Performs supervisory visits per agency standard on home care aides.
  • Teaches, supervises and counsels the patient and family to meet patient and family needs identified in the plan of treatment.
  • Participates in discharge planning when appropriate.
  • Maintains coordination with all other disciplines as needed for patient care.
  • Provides nursing support to patients and families.
  • Participates in the on-call schedule as shared by agency nurses.
  • Utilizes appropriate communication lines to problem solve personal and departmental issues.
  • Accepts other duties as assigned, which may vary based on agency needs.

Qualifications:

  • RN required license for Washington and Oregon.
  • TB screening.
  • One year of skilled nursing and wound care is preferred.
  • Proof of COVID 19 Vaccination or bona fide medical/religious exemption.
  • Current CPR certification.
  • Valid driver’s license, reliable auto, and proof of auto insurance.


Job Type: Full-time

Salary: $33.00 – $60.00 per hour
