RN Care Manager – HIV and Addictions Medicine
Schedule: Monday – Friday 8am – 4:30 pm
RN Care Manager with our HIV and Addictions Program.
This role includes working alongside an exceptional care team at Mason Square Neighborhood Health
- Care Management and engagement of persons living with HIV and Addictions receiving medical services a primary care setting.
- Provides independent quality triage and patient care utilizing strong assessment skills, point of care testing and quality assurance checks.
- Establishes plan of care, sets priorities and implements the plan of care with patient/family and team/provider input.
- Builds on knowledge and experience to include more diagnoses and complicated medical patients.
- Triages and coordinates patient care ensuring access to care and patient success.
- Promotes health and wellness for individuals and groups of patients.
- Develops patient trust and engages the patient/ family in care and treatment. Understands social and cultural differences that might impede patient care
What you will need:
- Massachusetts license as an RN and BLS CPR certified.
- Minimum of 2 years of nursing experience
- Experience in HIV, and or Addictions specialty strongly preferred
- Must be self driven with great engagement and communication skills

Pay: $52,578.00 – $98,307.00 per year
