CASE MANAGER – State of Arizona – Kingman, AZ

The Department of Economic Security, Division of Developmental Disabilities (DDD), is seeking an experienced and highly motivated individual to join our team as a Case Manager 2. DDD provides individuals with developmental disabilities, and their families, services and supports that are flexible, high quality, and member driven. The Case Manager 2 serves members with developmental disabilities, e.g., Autism, Cerebral Palsy, Intellectual Disability, Epilepsy, etc., by supporting them and their families to lead the healthiest, most self-sufficient, productive lives within their capabilities.

Job Duties:

  • In compliance with requirements, drive vehicle to conduct on-site visits to members/families homes, schools and program sites to develop relationships and family dynamics; discuss service needs and supports; develop and write the Individual Support Plan (ISP); document individual progress and concerns; and complete appropriate referrals to community agencies.
  • Research community for available resources to meet needs identified in ISP and to inform member of resource options. Facilitate meetings and program visits to assist with member/family choices. Complete referrals for assessed/covered services.
  • Develop, review, monitor and evaluate Individual Support Plan (ISP) in accordance with mandated timeliness. Approve type and level of assessed services within approved guidelines; and release client funds.
  • Collaborate with community agencies to provide assistance and support to members and families in crisis. Attend meetings with members/families, service providers, and others as required. Identify and resolve issues/conflicts.
  • Write comprehensive progress notes and maintain case records. Prepare a variety of reports and complete special projects, as required.
  • Compile information from a variety of sources in order to make recommendations for eligibility determination and re-determinations of eligibility. Confer with supervisor, attend staff meetings, training and other meetings, as required. Statewide travel may be required.

  • Knowledge of Federal and State statutes and agency policies and procedures relating to the care and assistance for individuals with developmental disabilities.

  • Knowledge of principles and practices of case management.

  • Knowledge of medical and behavioral health issues and terminology.
  • Knowledge of principles of applied behavioral interventions.

  • Skills in written and oral communication to convey a wide variety of information.
  • Skills in developing, evaluating, and maintaining member service plans.
  • Skills in interviewing, counseling, and interacting with members and family members.
  • Skills in conducting needs assessments.
  • Skills in problem identification and resolution.
  • Skills in interpersonal relations as applied to contacts with members and representatives of other governmental and community organizations and the public.
  • Skills in conflict resolution.
  • Skills in the use of various software (Microsoft Word, Excel, etc.).
  • Ability to work independently and within diverse work teams.
  • Ability to elicit accurate data from a variety of sources.
  • Ability to absorb, interpret and assimilate large quantities of written material.
  • Ability to organize and manage case load
  • Ability to document case files and information within required/mandated timelines.
  • Ability to maintain confidentiality.
  • Ability to drive a motor vehicle.

Pre-Employment Requirements:

  • In-state travel is required. This position requires possession of and ability to retain a current, valid state-issued driver’s license appropriate to the assignment. Employees who drive on state business are subject to driver’s license record checks, must maintain acceptable driving records and must complete any required driver training (see Arizona Administrative Code R2-10-207.12.). Employees may be required to use their own transportation as well as maintaining valid motor vehicle insurance and current Arizona vehicle registration; however, mileage will be reimbursed.
  • Employee shall have a valid Level One fingerprint clearance card issued pursuant to Arizona Revised Statute 41-2758.07 in order to work with children and vulnerable adults.
  • Candidates for this position shall be subject to a search of the Child Protective Services Central Registry pursuant to A.R.S. 8-804.
  • As required by AHCCCS Contract No. YH6-0014, a Case Manager must be an Arizona licensed registered nurse in good standing OR possess a degree in social work OR possess a bachelor’s degree in psychology, special education, or counseling AND have at least one (1) year experience providing case management services to persons who are elderly and/or persons with physical or developmental disabilities and/or members determined to have a Serious Mental Illness (SMI) OR have a minimum of two (2) years’ experience providing case management services to persons who are elderly and/or persons with physical or developmental disabilities and/or members determined to have a Serious Mental Illness (SMI).
  • For case managers who will serve persons who are elderly and/or persons with physical or developmental disabilities and have been determined to have an SMI the requirement is as follows: 1. One (1) year of case management experience serving elderly and/or persons with physical or developmental disabilities, AND 2. Two (2) years of case management experience serving members determined to have an SMI. Or in compliance with current AHCCCS Contract No. YH6-0014 contractual requirements.

  • Critical Services Premium Pay Stipend

This position may be eligible to receive a temporary stipend of 10% in addition to base pay beginning in December 2021. If eligible, the temporary stipend amount will be calculated using the employee’s base rate of pay and will be reflected in each paycheck. It is anticipated that this temporary stipend will be available through June 24, 2022.

Retirement:

State employees are required to participate in the Arizona State Retirement System (ASRS), the State sponsored retirement contribution plan and the Long-Term Disability (LTD) program after a 27-week waiting period.
