Nurse Practitioner – Stockton, California – Lightyear Health (External Job Board Postings) – Stockton, CA

Lightyear Health is looking for an empathetic and tech-savvy Nurse Practitioner (4 days during the weekday) to become part of our growing company that improves the functional independence and quality of life for aging patients. This role comes with the flexibility to work around your schedule, as there are no overnight calls or weekends.

To help us achieve our mission, we are looking to hire a Nurse Practitioner with at least one year of experience who envisions a world where every aging patient has access to the world’s best healthcare. This is a flexible role serving patients in the Stockton and Modesto area and will report to our Director of PM&R APPs.

As a key member of our PM&R team, here’s what you’ll do:

Key Outcomes:

  • Conduct roughly 15 in-person visits with patients daily in skilled nursing facilities, focused on quality of care.
  • Work with Physical Medicine and Rehab physicians to develop care plans and treatment recommendations for pain management and the improvement of functional and behavioral health in the geriatric populations.
  • Coordinate care and make treatment recommendations by attending weekly team meetings at skilled nursing facilities.

The most important skills for the role are:

  • At least 1 year of experience as a Nurse Practitioner
  • Nurse Practitioner licensed in California
  • Empathy and a genuine interest in providing care for a geriatric population
  • Strong interpersonal skills, high integrity, and collaborative approach with patients, their families, and the care team to accomplish life-changing outcomes

In addition to the above, it would be great if you also have (but not required):

  • Specialty certification in Gerontology
  • Experience working in SNFs or acute rehab setting
  • Experience in geriatrics, orthopedics, neurology, palliative care, pain management

Anticipated Salary Range

We’re committed to an inclusive, consistent, and equitable approach to pay and anticipate that this position will earn roughly $110k-$115k. We’ll cover all additional details in the course of the interview process. The exact salary will depend on one’s census size, which is typically 15-18 patients per day. Providers who meet our rigorous standards for clinical care and are able to efficiently see more patients can see earnings increase proportionally, and all team members receive a stock option grant.

Practitioners at Lightyear Health are responsible for meeting and complying with our credentialing verifications processes, clinical standards, health screenings, and vaccination policies at all times. It is the responsibility of the practitioner to ensure that your licenses, certifications, and credentials are kept up to date and in good standing.
