JOB TITLE: Hospital Care Transition Coordinator
EMPLOYMENT TYPE: Full-time
SCHEDULED HOURS: 40 hours/wk (Monday thru Friday 8:30-5pm)

SITE LOCATION: Primary site location Enfield, CT. May require occasional travel to Manchester, CT.

OBJECTIVE OF POSITION:
To provide high quality medical and behavioral health care coordination services to adults and or children enrolled in CHR’s Behavioral Health Home.
By:

  • Providing high quality hospital transition care and other medical and behavioral health care coordination services to CHR’s adult and child Behavioral Health Home (BHH) enrollees.
  • Developing and maintaining good working relationships with medical and behavioral health hospital inpatient units, emergency departments and community medical providers to improve healthcare outcomes for CHR’s BHH clients.

ABOUT BEHAVIORAL HEALTH HOMES:
BHH provides medical care coordination & health and wellness support to identified existing CHR clients with significant behavioral health and chronic health needs such as Diabetes, Obesity, Asthma/COPD, Coronary Artery Disease, Hypertension and Congestive Heart Failure
DUTIES & RESPONSIBILITIES:

  • Works to provides high quality hospital transition care and other medical and behavioral health care coordination services to CHR’s adult and child patients with SPMI and/or chronic health conditions who meet Behavioral Health Home (BHH) criteria.
  • Develops and maintains good working relationships with medical and behavioral health hospital inpatient units, emergency departments and community medical providers to improve healthcare outcomes for CHR’s BHH clients.
  • Works to identify BHH clients being admitted to and discharged from medical and behavioral health inpatient units.
  • Coordinates communication between medical and behavioral health care providers to provide proper scheduling of follow up appointments, medication reconciliation, arranging or providing client transportation, and other services as needed.

  • Comprehensive care management
  • Care Coordination
  • Health promotion
  • Comprehensive Transitional Care
  • Patient and Family Support,
  • Referral to Community Support Services
  • Works to help reduce health care disparities whenever possible. Advocates for clients’ rights and access to services
  • Maintains documentation in compliance with agency and program standards.
  • Participates in staff meetings, supervision, trainings and other meetings as assigned.
  • Additional duties as required.

QUALIFICATIONS:
Education: **BA in human services required.
Experience: 2 years experience in mental health setting preferred.
Licensure/Certification/Registration: Valid driver’s license. CPR/First-Aid Certification within 6 months of hire.
