The Nurse Practiitioner supports all aspects of patient care, including diagnosis, treatments and consultations. They provide general and preventative care, interventional care, order lab tests, educate patients and their caretakers and prescribe medication. They deliver role patient care through in-home visits, telephonic outreach and consultations, and interdisciplinary team activities.

The Nurse Practitioner is a licensed practitioner who works under a collaborative agreement (protocol) with a supervising physician. The nurse protocol is a written document in which the physician gives the NP authority to perform medical acts and agrees to be available for immediate consultation if necessary. The Nurse Practitioner is responsible for managing health problems and coordinating health care for the Optum at Home patients in accordance with State and Federal rules and regulations and the nursing standards of care. This includes (but is not limited to) assessment of health status, diagnosis, development of plan of care, implementation of treatment plan, ongoing evaluation of patient status and response to the plan of care, and ordering drugs, treatments and diagnostic studies. Clinical management is conducted in collaboration with other care team members.

Primary Responsibilities:

  • Perform age-appropriate history and physical for complex and chronically ill patients with the frequency established in the model of care
  • Complete initial, acute, follow-up and post-hospital visits to address gaps in care
  • Provide acute, follow-up and post-acute stabilization evaluation to engage resources and strategies to address medical, functional, and social barriers to care
  • Work closely with mental health clinicians to help bridge the gap between mental and physical health
  • Consult with the patient’s PCP, specialists, or other health care professionals as appropriate
  • Assess patient needs for specialty referrals and make appropriate referrals
  • Order and interprets diagnostic tests relative to patient’s age-specific needs
  • Prescribe appropriate pharmacologic and non-pharmacologic treatment modalities
  • Implement interventions to support goals to regain or maintain physiologic stability; monitor the effectiveness of interventions
  • Facilitate the patient’s transition within and between health care settings in collaboration with the primary care physician and other treating physicians
  • Completely and accurately document and code for patient conditions
  • Provide patients and family members with counseling and education regarding health maintenance, disease prevention, condition trajectory, diagnosis, treatment, and need for follow up as appropriate during each patient visit
  • Verify and document that the patient understands diagnosis, treatment and follow up recommendations
  • Facilitate understanding of patient/POA goals of care to support development of a comprehensive plan of care
  • Actively participate in organizational quality initiatives
  • Participate in peer support and mentoring activities
  • Participate in collaborative multidisciplinary team meetings to optimize clinical integration, efficiency, and effectiveness of service delivery
  • Maintain credentials essential for practice, to include licensure, certification and CME

Required Qualifications:

  • Graduate of an accredited Nurse Practitioner Program
  • Current Advanced Practice Registered Nurse (APRN) Licensure OR the ability to obtain
  • Board Certified through the American Academy of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC) OR the ability to obtain
  • Active, unrestricted nursing license in good standing in Arizona
  • Travel 100% of the time

Preferred Qualifications:

  • 2+ years of experience in practice as a Nurse Practitioner
  • Effective time management and communication skills
  • Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs
  • Advanced knowledge of and experience with symptom management
  • Ability to work with diverse care teams in a variety of settings
  • Experience working with patients in non-clinical settings
  • Experienced computer skills, including Electronic Medical Record
