Behavioral Health Intake Coordinator – PRN Nights – Grandview Medical Center – Birmingham, AL

Performs biopsychosocial assessments of behavioral health patients presenting to the hospital setting, including patients having complex psychosocial, mental health, or chemical dependency conditions. Determines whether patient meets admission criteria for inpatient hospitalization, and arranges such admission if appropriate. Facilitates and coordinates development of an outpatient discharge plan of care for high risk patient populations. Works with Behavioral Health nursing leadership to manage admissions, discharges, and transfers to maintain throughput of psychiatric patients throughout the hospital. Receives referrals of individuals who are of an at-risk population from case managers, staff nurses, team members, physicians, and external referral sources. Candidates with a background in either clinical social work or psychiatric nursing may apply.

Nursing: Minimum of ADN or BSN nursing degree from an accredited school of nursing required. Social Work: Master’s degree in social work from a CSWE accredited school of social work required.
