We’re looking for a program manager who is passionate about maintaining Carbon Health’s innovative future of healthcare. In this role, you’ll oversee our existing Primary Care operating model. Use your operational, creative and analytical skills to maintain workflows, support the economic model, placing constant rigor around continuous improvement and optimization. You’ll also lead the design, development and implementation of new primary care services, initiatives and pilots, working cross functionally with Clinic Operations, Program Management, Clinical teams, Product, Marketing and Finance. Additionally, you will support our Primary Care Central Support Specialists and the Extended Care Team enabling their success.

  • Ensure continuity across all Primary Care programs and channels, maintaining a cohesive operating model, managing dependencies, deliverables and risks
  • Work closely with different internal stakeholders to design new primary care services and champion integration into existing primary care programs
  • Ensure alignment and communication across the program/project team(s). Address barriers and blockers to execution.
  • Lead ongoing optimization and expansion of our current Primary Care programs across all clinic types and primary care channels
  • Support the economic model, analysis, reporting and share outs with internal stakeholders
  • Deep healthcare and care delivery experience, ideally with a health system, payer, or primary care startup
  • Experience designing new programs and integrating into existing programs
  • Bachelor’s degree with a minimum of 7 years of related experience. Master’s degree a plus
  • Ambidextrous skill sets: analytical and operational with an emphasis on getting things done
