Service Coordinator (TBI & NHTD) – Community Options, Inc. – Syracuse, NY

Community Options, Inc. is a national non-profit agency providing services to individuals with disabilities in 10 states.

We are seeking a Service Coordinator in Syracuse, NY.

The Service Coordinator uses best practices and technology within a community-based setting to assist, train, and supervise individuals in daily living activities and socialization skills. The Service Coordinator assures that the service plan is implemented appropriately. This includes monitoring the delivery and follow-up of waiver and non-waiver services. The SC serves as an advocate and liaison for and on the waiver participant’s behalf. They support the participant in becoming an effective self-advocate and problem solver. The SC assists the waiver participant in maintaining benefits including but not limited to Medicaid, SSI, and SNAP.


Requirements:

  • Bachelor’s degree with three (3) years of experience providing case management/service coordination, information and referral services, and service brokering of community-based services for individuals with disabilities or Traumatic Brain Injuries
  • TBI and NHTD overview training
  • Valid driver’s license and satisfactory driving record
  • Successful completion of required trainings
  • Flexibility to deal with people at a variety of levels, both internally and externally
  • Ability to present facts and recommendations effectively in oral and written form
  • Excellent crisis and time management skills
  • Excellent interpersonal and communication skills
  • Ability to establish a comfortable and supportive relationship with individuals receiving supports
  • Ability to meet deadlines to ensure participants’ continued eligibility in the waiver program
  • Ability to lift 50 pounds (weight of average manual wheelchair when chair is empty)

Responsibilities:

  • Gathers the necessary information for the applicant to be approved as a waiver participant
  • Secures a residence in the community that meets the individual’s welfare and safety needs
  • Creates and implements the service plan, making revisions every six months
  • Develops service plans and ensures consumer assessments are completed to determine if goals and objectives are being met
  • Supports and encourages the waiver participant to increase his/her ability to problem solve, be in control of life situations, and be as independent as possible
  • Identifies, develops, and coordinates community resources and support
  • Maintains all documentation of service delivery and billing
  • Assures that the assessment of need for a nursing facility level of care will be initiated on the anniversary of the initial determination (UAS).
  • Initiates a re-evaluation of the level of care when the waiver participant has experienced a significant improvement in his/her ability to function independently in the community
  • Attends conferences, seminars, and trainings as necessary
  • Other duties as assigned by the SC Supervisor

Working Conditions:

  • Work a flexible schedule as determined by supervisor
  • Work will take place in community and individual’s homes
  • Occasional overtime required
  • While performing the duties of the job, employee travels by automobile and is exposed to changing weather conditions
  • May be exposed to various medical conditions and communicable diseases
