Outreach and Engagement Specialist- TREE – Native American Connections – Phoenix, AZ

Improving the lives of individuals and families through Native American culturally appropriate behavioral health, affordable housing, and community development services.


POSITION SUMMARY
:

The Outreach and Engagement Specialist will work collaboratively with the Behavioral Health and Housing teams to build working relationships and identify areas of outreach to locate disconnected youth for the TREE grant. The Outreach and Engagement Specialist will engage and connect clients to resources both within the organization and externally in the community. Note: This is a grant funded position. 5 years funding/ 2.5 years left in the grant cycle.


RESPONSIBILITIES
:

  • Works with Youth and Family TREE Project Director to ensure grant goals and objectives are implemented and remain on target through the duration of the grant.
  • Identify and outreach uninsured and underinsured youth ages 16-25 in need of substance use treatment and/or education, prevention or connection to services.
  • Schedules grant intakes and behavioral health intake appointment for the TREE staff.
  • Engage and support clients to move from an emergency care model to a preventative/primary care model of health and wellness.
  • Completes insurance screenings for potential TREE clients and updates internal tracking system to reflect this work.
  • Establishes a partnership for potential referrals and engagement with the Phoenix Union High School district.
  • Tables at target aged youth events at local universities, colleges, and technical schools/program.
  • Delivers TREE program presentation to the Correctional Health Services Mosaic Program.
  • Develop and maintain collaborative working relationships with community based organizations, social service agencies and other services providers in order to refer and coordinate services across multiple agencies including but not limited to Phoenix Indian Medical Center, Circle the City, Phoenix Indian Center, Native Health, Native youth organizations and local hospitals and schools.
  • Provides structured outreach in the community at least 60% of the time, including services being provided on-site with Homebase Youth Shelter.
  • Actively screen and assess client needs around Social Determinants of Health (SDOH). Provide referral as needed to increase health and wellness.
  • Provides education or teaching regarding use of appropriate healthcare utilization; assist with assignment of an integrated medical home and coaching regarding health literacy and healthcare provider communications.
  • Maintains and organizes outreaching tracking spreadsheets for data collection on behalf of the grant.
  • Participate in a variety of health education opportunities, including facilitation of classes/groups and one-on-one encounters with clients as needed.
  • Increase visibility of TREE program in the community via participation in youth-related events, presentations to community agencies/schools, and posting to NAC social media accounts.
  • Will work with NAC’s Marketing Specialist to create virtual/social media posts using innovative strategies to provide virtual outreach as needed due to pandemic.
  • Prepares data for ongoing grant deliverables on outreach and engagement activities.
  • Participates in weekly department team meetings across various sites to discuss client barriers, accomplishments and outcomes.
  • Addresses the client as a whole, encouraging mind, body and spiritual connectedness.
  • Support and actively engage in practices of inclusion through Cultural and Linguistically Appropriate Services (CLAS) Standards.
  • Other duties as assigned.


EDUCATIONAL REQUIREMENT
:

  • Associates degree or Bachelor’s degree preferred.

WORK EXPERIENCE/SKILLS REQUIREMENT:

  • MS Office proficiency.
  • Strong interpersonal skills with an ability to work effectively with a wide range of people, teams, managers, supervisors, and vendors.
  • Demonstrate excellent written and oral communication skills.
  • Must demonstrate critical thinking, problem-solving and organizational and time management skills.
  • Demonstrated working knowledge of the economic, educational and social problems of Native Americans and referral sources available.
  • Possess and maintain a valid Arizona driver’s license with proof of insurance and must obtain Fingerprint Clearance within 90 days of hire
  • Must have working knowledge of care management, community resources and resource/utilization management.
  • Bilingual-Spanish desired.
