Ability HomeCare is a large Pediatric Home Health Agency seeking a dedicated RN case manager with a solid work ethic for our PDN Case Manager role. The ideal candidate will have 2 years of home health experience, preferably in a pediatric environment, management skills, be personable, flexible, energetic, detailed, organized, possess excellent writing and communication skills, and the ability to communicate tactfully to a variety of staff and clients.
The role will include, but not be limited to completing initial and re-evaluations for PDN clients, oversight of plans of care, management of each assigned case, training of nurses, shared on call, and act as a support to the Director of Nursing and the entire PDN team. Candidate MUST be a licensed RN.
