OVERVIEW
We are a starting hospice and palliative care provider who are in search for an experienced hospice Registered Nurse. The hospice RN is responsible for providing care in the home to maximize the patient and family’s quality of life. This candidate must have excellent phone / text / email communication with the team and be someone who is driven toward quality outcomes and teamwork.
Patient assignments cover between South Bay area, Long Beach, Norwalk, and Anaheim.
Responsibilities and Duties
The hospice RN’s goal is to work collaboratively to be an important part of our Interdisciplinary Hospice Team and help Epione Hospice, Inc. drive toward excellent outcomes for our patients and strategic partners.
The hospice RN has the following responsibilities:
- Admit patients to hospice care through Explanation of Benefits and Comprehensive Nursing Assessment (Per Admission Compensation Rate)
- Performs Supervisory Visit with Health Aides or LVNs to direct and review hospice care being provided accordingly to patient needs
- Respects patients and families
- Educate the hospice concept and the benefits of program and services to patients and families
- Participate in IDG/IDT and updating of the Plan of Care in HospiceMD (EMR)
- Provide our patients and families the best care possible
Qualifications and Skills
- Must possess a current and valid California RN license
- 3 years or more of experience in Palliative Care, Home Health or Hospice is required
- Must successfully pass required background checks
- Must possess a valid driver’s license, provide proof of insurance, and have reliable transportation
- Friendly, collaborative, outgoing personality
- Experienced in Hospice MD web platform for documentation is a plus
- Must have excellent phone | text | email communication with the team
- Bilingual in any language a plus
Job Type: Part-time
Pay: $85.00 – $250.00 per day
