Project Manager (FT) Population Health – D-H Lebanon-D-H Clinic – Lebanon, NH

Purpose:

Manages or assists with work on assigned projects through the application of project management principles. Manages or assists with the identification and prioritization of opportunities and risks within projects. Exposes directors, managers, supervisors, and front line staff to project management and process improvement principles and tools. Fosters and supports change within the organization.

Key Responsibilities:

1. Works closely with the sponsor and key stakeholders to develop effective project management approach.

2. Collaborates with the project sponsor and project team members to define and document the project charter.

3. Clearly articulates and shares project objectives and scope with appropriate parties.
4. Develops project plans, goals, deliverables, and tailored work plans.
5. Identifies assumptions, constraints and dependencies for the project.
6. Manages project communications and relationships with stakeholders.
7. Coordinates with project leadership to identify, assemble, manage, monitor and motivates cross-functional project teams. Monitors activities of all project team members to meet objectives.
8. Conducts quantitative project risk assessments. Collaborates with project leadership to develop plans to monitor, minimize, and mitigate project risks.
9. Monitors the project budget.
10. Defines and manages the project change control system.
11. Manages and documents project meetings and facilitates problem-solving.

12. Conducts project presentations independently or in collaboration with project leadership, or as part of curriculum delivery.
13. Keeps leadership and the project team apprised of project progress through routine progress reports. Provides ongoing updates to the project management plan.
14. Supports sustainability of achieved project benefits through a formal hand-off process and control plan implementation.
15. Assesses project environment and recommends best approaches to change management.
16. Explains project management processes, tools, and techniques to the project team.
17. Participates in ongoing project management curriculum development.
18. Networks and collaborates on methodology and tools with other project managers.

19. Performs other duties as required or assigned.


Minimum Qualifications:

 Bachelor’s Degree with 3 years of experience in project management required.

Strong analytical problem-solving experience.
 Demonstrated competency in project management skills within a complex, cross-functional environment.
 Excellent communication, organizational, decision-making, and leadership skills, with the ability to work independently and as part of a team.

Must be skilled in the use of MS Word, Excel, PowerPoint and Outlook.
 PMP certification or equivalent preferred.
 Previous healthcare industry experience preferred.

Required Licensure/Certification Skills:
 Obtain PMP Certification within 18 months of hire and continue to maintain that certification.
