Senior Health Nurse Practitioner – East Peoria and Washington, IL – UnityPoint Health – Peoria, IL

Overview:

UnityPoint Clinic is actively recruiting for a Senior Health Nurse Practitioner or Physician Assistant to join our Central Illinois team.

Overview:

• Independent role for a Provider caring for patients in a nursing home setting
• Established practice serving nursing home patients across Central Illinois
• Work autonomously in two nursing homes, while collaborating with four Physicians and a team of eight other Advance Practice Providers
• Full training, including shadowing your collaborating Physician and other Providers, plus extensive Epic EMR training and support

Responsibilities:

• Medical management of adult patients in a nursing home setting
• Handling acute and chronic medical conditions, new admissions, and rounding
• Average 15 patients/day; coverage for 2 local Nursing Homes in Tazewell County, with only one building scheduled per day (some occasional coverage of other facilities in Peoria or Tazewell County if needed to cover vacations)
• Weekday schedule M-F 8am-4:30pm with flexibility to complete dictation/EMR at home, if desired, after patients for the day have been seen
• Rotating call schedule includes 1 weeknight per week and 1 weekend per month, covered by phone
• Ability to work as a team member to improve the health of the people and communities we serve
• Provide the best outcome for every patient every time

Qualifications:

• Ideal candidate will have 1-3 or more years of experience in patient care in a nursing home. New graduate NP’s or PA’s are welcome to apply if they possess significant nursing home experience.
• Board Certified Nurse Practitioner or Physician Assistant
• IL License with no restrictions
• Eligible to work in the United States without employer sponsorship
